It will be my pleasure to look after your pregnancy. Please see below a list of the terms of our engagement in respect of your antenatal care. 1. Provision of the Service I am contracted to undertake antenatal care only and to undertake one postnatal visit. Delivery will be undertaken by the NHS and you will be considered an NHS patient of the hospital. In the majority of circumstances, I will be either directly involved or leading the management of your delivery. Care during labour and the postnatal period is provided out of courtesy. The only exception to this is a patient whose caesarean section is covered under the terms of their insurance policy. Such patients are considered fully private during their delivery. If a patient has this benefit under their insurance policy this must be brought to my attention by no later than the second visit. Routine antenatal blood tests will be explained to you and performed with your consent, at your second visit, at 30 weeks and at 34 weeks (only if you are rhesus negative). Blood pressure and urine will be tested at every visit. You will be scanned at every visit and you will receive printouts and a DVD recording at 25 weeks. At 25 weeks, you will also see 3D images of your baby. The quality of these images will vary according to fetal position. I am not liable for any complications which arise and which are outside of my control during (i) any antenatal in patient hospital stays; (ii) labour or (iii) postnatal hospital stays. Such M-6907442-1 1 complications will be dealt with in accordance with the NHS indemnity scheme as an NHS patient. 2. Delivery Throughout our engagement, clinical safety of mother and baby are of paramount importance. Delivery shall occur in the Royal Jubilee Maternity Hospital, as an NHS patient. It is not possible for me to be involved in your care if you attend or are brought to another hospital or if you are transferred to another hospital antenatally due to neonatal requirements for a possible preterm delivery, which may only be available elsewhere. Whilst it is my aim that either I, or one of my nominated deputies (in my absence) will either personally attend or supervise the delivery, this may not always be possible due to other NHS, personal commitments or time constraints which determine that delivery either occurs too soon or must be effected sooner due to particular emergency circumstances. In order to facilitate my being present for your delivery, it is my practice to offer you induction of labour at around 41 weeks, on a convenient day for both of us. I will only offer this if you are agreeable and if the baby is in the appropriate position for induction of labour. Some women may wish to have a planned caesarean section from the outset. I am happy to consider such requests provided I have had a chance to discuss the issues fully with you. If we proceed, the caesarean section will be performed around 39 weeks. Only when a patient is fully insured for delivery by a caesarean section, is care in the NHS hospital deemed to be fully private. In such circumstances, the consultant obstetrician, consultant anaesthetist and the hospital will bill the insurance company for the elements of the delivery. Such fees are in addition to the antenatal fee described at paragraph 4 below and shall be borne by the insurance company. It is your responsibility to provide me with the details of your insurance policy and authorisation number beforehand so that arrangements can be made in advance. In emergency circumstances, despite the correct preauthorisation, insured patients may be delivered by the on call team if clinical circumstances require expediency or if a consultant which is not registered by the insurance company is on duty. 3. Care at the Royal Jubilee Maternity Hospital No fee will be attributable to any care provided in the Royal Jubilee Maternity Hospital which includes any antenatal emergency attendance, admission or delivery. This care will be provided to you as an NHS patient. Either myself or one of my nominated deputies (in my absence) will endeavour to supervise or lead the care in any antenatal emergency attendance, admission or delivery, however this is not guaranteed and will be provided out of courtesy. In the event that either myself or a nominated deputy are unavailable, the emergency team on call will provide the necessary M-6907442-1 2 care. You may wish to have a single room during your stay in hospital. This is an arrangement between yourself and the hospital and is outside the terms of this agreement. Room allocation is on a first come first served basis depending on availability. Room requests for elective caesarean sections can be requested the day before admission and are assigned on the morning of admission, if available. Rooms for labouring women are assigned shortly after delivery. Once you have accepted a bed in a general ward, you cannot be transferred to a single room, if one becomes available later. 4. Fees The fee for the antenatal care is £3100.00 and must be paid in up to 4 instalments as follows: i) £200 at the first visit ii) £1400 by the 20th week of your pregnancy iii) £900 by the 30th week of your pregnancy iv) £600 by the 35th week of your pregnancy. Payments can be made at reception or in the case of credit /debit card payments, by telephone in advance of the relevant appointment. Post dated cheques supported by a cheque guarantee card are also accepted but must have ‘cleared’ in advance of the relevant appointment. Please note that credit card payments incur up to a 3% administration charge. If you are paying by cheque for instalment (i), please make this payable to ‘352 Medical Limited’. For instalments (ii to iv) please make this payable to ‘Dr Stephen Ong’. Failure to make a payment (including a failure of any payment to clear) in accordance with the above will result in your immediate transfer to NHS care under a different consultant. The undertaking of this transfer will be your responsibility. If delivery occurs prematurely in the Royal Jubilee Maternity Hospital or another NHS hospital, then the fee charged will be a proportion of the total fee for that period of gestation as follows: a) delivery between 20 and 30 weeks (£1600 + a proportion of £900) b) delivery anytime after 30 weeks (the total fee is due) M-6907442-1 3 Other OPTIONAL fees (payable at reception at the time of the request): Amniocentesis Parentcraft £430 (includes laboratory fees) £125 I/We confirm that I/We have read and understood the terms of engagement listed above, understand these to be legally binding and agree to comply with the said terms of engagement Patient Name: …………………………………… Signature ……………………………….
