The application will be formally reviewed and considered by the CCG / NHS England Director of Commissioning Office. On approval, an allocation of funds will
be made available to the Co-commissioning CCG / DCO team to effect reimbursement to the practice.
NHS England will not be able to accept applications and supporting documentation after 30 November 2017 due to the short term nature of the
Fund.
