JOB SUMMARY
The post holder will work as part of a dedicated pharmacy team to provide and develop highly specialist clinical
pharmacy services to Liver. The post holder will liaise between wards and departments and pharmacy to ensure
that pharmacy services are tailored to meet the Divisional needs wherever possible.
The post holder will participate in the following services:
 To act as the deputy for the Clinical Pharmacy Team Leader, Liver where necessary (in the absence of the
Team Leader and Senior Liver Pharmacist, 8a)
 To act as the Discharge Coordinator for the Liver Pharmacy Team, managing the Pharmacist discharge
prescription writing service (“drug lists”) and writing “drug-lists” for patients across the specialty
 To consistently lead on the provision of clinical pharmacy services to a sub-specialty within Liver and ensure a
high level of clinical pharmacy input to the Liver wards, Liver Theatres and Outpatient department
 To participate in promoting high quality, cost-effective, patient-focused pharmaceutical care
 To act as a clinical role model in Liver and supervise and train senior pharmacists and rotational staff at ward
level
 In liaison with the Team Leader - to manage FP10 prescribing in Liver Outpatients




Promotion of safe drug use within the specialty
Development, implementation and audit of local guidelines
Auditing drug usage in the Division with the aim of promoting cost effective prescribing
Education and training of healthcare professionals within and outside the Trust relating to medicine use in liver
disease
 To participate in practice and clinical research and audit
 Ensuring seamless pharmaceutical care on discharge from Liver wards, liaising with other healthcare
professionals (primary and secondary care) where necessary
 To work with the Clinical Pharmacy Team Leader to extend the boundaries of professional practice within the
Division
GENERAL
Strict confidentiality should be maintained regarding all matters pertaining to patients. All work will be carried out
according to the laid down procedures and standards of professional practice in the Department; however, staff
are expected to develop their own safe systems of work to ensure a high professional standard of work conducive
to patient safety.
MAIN DUTIES & RESPONSIBILITIES
POLICY/SERVICE DEVELOPMENT
 To propose and implement policies, guidelines and procedures designed to improve the pharmacy service to
patients and the Division in conjunction with the Team Leader
 To comment on policy and service changes and plans within the clinical pharmacy service and to contribute to,
and participate in, pharmacy service developments in line with departmental and Trust policy
 To develop, implement and audit prescribing guidelines for use within the Division in conjunction with the Clinical
Pharmacy Team Leader, Liver
 To deputise for the Pharmacy Clinical Pharmacy Team Leader, Liver, where necessary.
 To assist in the development, implementation and audit of evidence-based prescribing guidelines in Division
 To ensure services provided are in line with local and national policies, guidelines and frameworks
 To contribute and adhere to clinical pharmacy service standards
PATIENT/CLIENT CARE
 To act as a specialist practitioner in Liver
 To promote and deliver pharmaceutical care specifically for patients within the Division and more generally for
all patients
 To actively participate in patient education schemes
 To provide highly specialised evidence-based clinical advice and information to nursing and medical staff on the
care and treatment of patients
 To actively participate in key multi-disciplinary ward rounds and clinical meetings within the Division
 To contribute to and participate in the clinical/ward pharmacy service
 To ensure appropriate and timely supply of medication to patients in conjunction with dispensary staff
 To liaise with other pharmacists/healthcare professionals to ensure efficient transfer of clinical and medicines
management issues
 To participate in discharge planning where appropriate including the writing of discharge prescriptions
 To ensure appropriate and timely supply of medication to patients, including participation in the POD scheme
 To liaise with other pharmacists to ensure efficient transfer of clinical and medicines management issues
FINANCIAL/RESOURCE MANAGEMENT
 To identify areas of potential cost-savings within the Division, and to implement systems to realise savings
and contain prescribing costs
 To support the development and implementation of policies to control drug expenditure and ensure
cost-effective prescribing.
 To educate other healthcare professionals on the costs associated with drug therapy and promote the use
of evidence-based, cost-effective prescribing within the Division
 To provide accurate and timely data to the Trust Finance department regarding use of high cost drugs to
ensure recouping of income from the PCT
STAFF MANAGEMENT & TRAINING
 To participate in the recruitment and selection of pharmacy students, pre-registration pharmacists, and junior
pharmacists as needed.
To assist in the training, supervision and assessment of junior pharmacy staff providing a ward/clinical
pharmacy service to wards within the Division.
 To plan activities for, supervise and train pre-registration pharmacists and pharmacy students where
appropriate
 To supervise and support the day to day activities of technical staff providing ward based pharmacy services if
required
 To participate in the in-service clinical pharmacy training of all pharmacy staff.
 To undertake CPD to ensure that knowledge is continually updated
 To participate in the training and development of medical, nursing and other healthcare staff, as appropriate
RESEARCH & DEVELOPMENT
 To regularly initiate or contribute to relevant practice research or audit projects within the Division and to
publish the results of such work, as appropriate.
 To participate in other research and audit programmes in the department as required
 In conjunction with the Clinical Team Leader, to develop, implement, monitor and evaluate evidence-based
protocols and guidelines
 To maintain and further develop specialist information on the use of drugs in clinical conditions relevant to the
specialty
 To publish and present the results and outcomes of own research and audits locally, nationally or
internationally as appropriate
 To proactively support the development of ward and clinical pharmacy services to Liver in order to advance the
safe, clinically effective and cost effective use of drugs specifically to Liver patients and more generally to all
patients.
 To liaise with clinical trial nursing and pharmacy staff with regards to monitoring and continuing supply of trial
medicines to patients locally and on national level
GENERAL RESPONSIBILITIES

Qualifications:
 Masters degree MPharm
 Registered Pharmacist GPhC (1 year competency assessed and examined
professional registration)
