Ward B1 - Clumber Unit Information: Patient, family, carer The information provided is designed to give you, your family or carer basic information about the ward and the service we provide. We aim to make your stay in hospital as comfortable and short as possible. Ward B1 - Clumber Unit Mental Health Services for Older People Ward B1 consists of two units, our aim is to provide a range of specialist services to those who present with mental health concerns. The Unit where you will be staying contains five assessment beds, the accommodation consists of five individual side rooms. You have access to showers and toilet facilities, TV lounge / dining room, a small garden (following assessment by the nurse in charge ) and activities under the guidance of an activities coordinator. On the Clumber Unit we treat each individual regardless of race, culture, spirituality, gender or sexuality with privacy, dignity and respect at all times, and an environment that is free from discrimination. We believe it is our responsibility to be able to meet the needs of you and each individual for whom English may not be the first language. The Multi-Disciplinary Team (MDT) are involved in your care, to ensure that the highest standard possible is reached by using skills in assessing, planning, treatment and reviewing, to meet your individual needs. 2 Clumber Unit offers Nutritional Intake Information and support for carers Physical state including mobility A caring and therapeutic approach, a safe and supportive environment which respects the dignity and rights of you the individual Sleep pattern Effects of medication. The allocation of a named nurse on admission An assessment of needs A coordinated approach to a planned discharge. The Multi-Disciplinary Team (MDT) includes: The ward manager, deputy ward manager, staff nurses, health care assistants, physiotherapist, activities coordinator, doctors etc. Qualified nurses: The Assessment of needs Are on duty at all times, they will admit and coordinate your care with you while you are in hospital, your individual needs will be identified and discussed with you and other Multi-Disciplinary Team members. Clumber Unit: We will be assessing the following and how they affect your ability to cope with your daily life. Mental capacity Confusion Social skills Behavior Dressing and personal hygiene. Observation levels: From admission you will be placed on nursing observations for the initial 24 hour period. This means that staff will visually observe you either constantly or at regular intervals. This is for your safety as part of your care. Nursing observations will be reviewed by the MDT Team after the 24 hour period. Staff are available at all times to discuss any concerns or issues that you may have. 3 4 Mental Health Act 1983 Telephone You may be admitted under a section of the Mental Health Act 1983 / 2007, the nurse in charge will give you written information regarding this, they will also read you the information if required to enable you to receive a better understanding. The nurse in charge will also assist you to appeal against your section if you so wish. Your relatives or carers can call the ward if they have any concerns. If your relatives or carers wish to speak to you please ask them to make arrangements with the nurse in charge prior to phoning. Your relatives should ring after 10.00am as this is a busy period on the ward and if they can be aware to avoid ringing at meal times. Informal patients If you are informal you have the right to withdraw consent to treatment. Due to the concerns the MultiDisciplinary Team (MDT) would be contacted for your safety to try and resolve the issue. If you still wish to leave the ward against doctors medical advice, documentation will be completed in line with the ward policy and procedure. If the MDT Team felt you needed to be in hospital a mental health act assessment may be arranged. If you have a mobile phone all charges will need to be P.A.T. tested. We request that you hand in your phone charger to staff for safe keeping. In consideration to other service users we request that all mobiles are on silent when not in use and for them all to be switched off before 10.00pm. Leave from the ward All leave from the hospital and time off the ward will be discussed and negotiated with yourself, and the MDT or the nurse in charge. If for any reason you do not comply to the above requests, due to the breach of confidentiality the nurse in charge will confiscate your phone and this will be put in safe keeping. Handover Confidentiality Handover takes place at the beginning and end of each shift. This ensures that each change of staff are fully informed about the care you and others may need and ensures the continuity of care provided. Personal information relevant to your care is held safely on the ward. The Multi-Disciplinary Team (MDT) has to have access to this information to discuss your individual plan of care. The NHS has strict guidelines about keeping personal information confidential. 5 Confidentiality is important to all, for this reason no pictures must be taken on the ward. 6 Violence / verbal abuse Fire procedure The Trust operates a zero tolerance policy. The following are some examples of behaviors that are NOT acceptable while you are on Trust premises and may lead to action under the policy. If the fire alarm sounds the staff nurse in charge will give directions immediately. An intermittent sound indicates we do not need to evacuate and a continuous ring indicates that the ward needs vacating safely and promptly. Violence Threats or threatening behavior Theft The fire assembly point is the outside car park area. The nurse in charge will coordinate any evacuation. Willful damage to Trust property Smoking Offensive sexual gestures or behavior A non smoking policy is in operation for all staff and visitors. Derogatory racial or sexual remarks Threatening or abusive language involving excessive swearing or offensive remarks Excessive noise e.g. loud or intrusive conversation or shouting. You may be exempt with an agreement from the Responsible Medical Officer (RMO). There is a designated area for smoking under supervision from nursing staff. Cigarette lighters are to be handed in on admittance to the ward for safe keeping. (When required the staff will provide you with a lighter which you may use upon negotiation and or supervision from the nurse in charge). After acts of violence / verbal abuse, discussions and decisions about your future care will be made in the Multi-Disciplinary Team (MDT) review. If you decide to stop smoking during your stay in hospital, smoking cessation, information and treatments can be provided. Any visitor who displays any of the above behaviors will be asked by the staff to leave the premises. Taken from Nottinghamshire Healthcare’s ‘Zero Tolerance Policy and Procedure’. Please ask a member of staff. 7 8 Visiting hours Transfer of patient to a medical ward Monday to Sunday 2.00pm to 4.00pm and 6.00pm to 8.00pm If the Multi Disciplinary Team (MDT) are concerned about your physical health, they will liaise with the medical team, and if required you may be transferred to the appropriate medical ward. Your relatives / carers would be notified as soon as possible. Please note there are only two visitors per patient allowed. All relevant personal information is kept confidential under the NHS Trust Policy, if you have any concerns please ask the manager / deputy manager / nurse in charge, please do not hesitate to ask we are there to help. There is a designated area for all visitors. All visits by children to the ward should be discussed and planned in advance with the ward manager or the nurse in charge. Children are not allowed on the ward but arrangements can be made for a room off the ward for your safety and comfort. Please ask your relatives to ring to make an appointment. Children should remain under the supervision of an adult at all times. Mixed gender Clumber Unit is a mixed gender ward. There are five individual side rooms, private facilities showers, toilet etc. Any preference you may have regarding privacy will be respected and catered for as far as the environment will allow. You, your relatives and carers are invited to make an appointment to see the ward consultant / ward doctor preferably after the first week of your admission. However we are flexible, if you would like to see medical staff before then, please telephone the medical secretary for an appointment. If you have any concerns please do not hesitate to contact the nurse in charge. Clothing requirements We encourage you to wear your own clothes during your stay on the ward, we would therefore ask you to bring a suitable supply of clothes and foot wear shoes / slippers for this purpose. We ask that you have a clean set of clothing for each day. If you have no clean clothes it may be necessary for you to be dressed in hospital attire until the clothing issue has been resolved. Although we do try to keep check on your property, unfortunately this is not always possible. Please be aware the ward has no washing facilities available. Please note the ward is not responsible for clothing that is lost or mislaid. We would like to thank you for your cooperation in this matter. Valuables Due to the nature of the ward, we recommend that any money or valuables which you may have are: Shower gel Hairspray Liquid soap Face cream Shampoo Brush / comb Perfume Deodorant Nail file Talcum powder Toothbrush and toothpaste or denture cleaner and pot. Kept by your relatives The hospital does not provide any items of toiletries. Please bring with you an adequate supply. Handed over to the nurse in charge and locked in the safe Electrical items The ward cannot accept any responsibility for any property lost (including clothing), which has not been safeguarded in this matter. Any electrical item (shavers, radios, etc) brought in for use in hospital must be booked in to be P.A.T. tested for safety by the hospital estates department. The item cannot be used until this procedure has been carried out. Patient toiletries - Male Shower gel Shaving equipment/razor (disposable) Liquid soap Deodorant Shampoo Aftershave Brush/comb Toothbrush and toothpaste or denture cleaner and pot. Protected mealtimes The purpose of protected mealtimes is to protect your meal time from unnecessary and unavoidable interruptions, this should ensure an environment conducive to eating. Staff can then provide you with support placing food first at meal times. 11 12 Research shows that patients who are not interrupted and receive appropriate service and support during meal times are happier more relaxed and eat more. The better nutrition a patient receives, the higher your chances are of recovering. Rest periods Rest periods have been initiated to enable you to take rest without interruptions. We have found that it helps with calmness and relaxation. Translator Food There is a translator available for any patient whose first language is not English. This service is available throughout the day and night if required. There is a leaflet on this service, please take one, any problems see the nurse in charge who will gladly help. Food that may be bought in includes, fruit, sweets, biscuits, these must be wrapped and in date. Any perishable goods you may wish to bring should be handed to staff so they can be stored in the kitchen. No other food should be bought in unless you have sought agreement from the nurse in charge. You can bring non-alcoholic drinks. Hearing If you have trouble with your hearing an audiology appointment can be made for you while you are on the ward. Any problems please speak to your named nurse. Please ask your relatives or carers to read the notice board for information. Infection control When entering or leaving the ward we ask that you use the alcohol gel located on the wall just outside the main door. Good hand hygiene helps reduce infection being brought onto the ward. To help reduce infection control we request that no flowers be bought on to the ward. Health and safety For the safety of the patients we operate an intercom system with a webcam in place where ward staff can check and identify all personnel wanting to gain access to the ward. Medication Red aprons are used when administering medication. In order to promote best practice, if you see a nurse wearing a red apron please do not approach him / her unless it is specifically relating to the drugs being administered, or in the case of an emergency. To ensure you receive the full attention you deserve please approach another member of staff. Any sharp objects / razors need to be handed in on admittance to the ward for safe keeping (you may access the items when required by requesting them from staff). All type of medication you are on must be handed to the nurse in charge. Any type of plastic bags need to be handed to the nurse in charge. 13 14 Church services Helpful local contacts There are services available every Sunday. Please ask the nurse in charge for more information. You would need to be escorted by a family member or carer. Activities We have a variety of ward activities to accommodate your individual needs, organised by the ward activities coordinator on a daily basis. We are able to provide a number of alternative therapies such as reiki, aromatherapy etc. Car park There are a number of ticket machines available around the hospital grounds. The car park facilities are Pay on foot / DISABLED PARKING for badge holders. There are disabled bays at the front of the entrance of the hospital. Refreshments available There are number of places that your relatives and or carers can buy refreshments when they visit you. There is a WRVS coffee shop which offers snacks and drinks away from the ward. The hospital also has a shop which is just through the main entrance to the hospi15 tal. The advisor will usually visit the person at home to assess their situation and needs. They can then offer advice and information on a range of topics such as home safety and security, home adaptations, mobility aids, transport, social activities and benefits.
