Latest progress of the North East Hampshire and Farnham Vanguard Thursday, July 9, 2015 Overview The momentum and pace continues to pick up for the programme. Nationally and locally anticipation has grown for project outcomes and Vanguard continues to become the focus with national bodies and the media. The project management office (PMO) is being approached (almost daily) with requests for speakers for national and local events such as conferences and community group forums. We want to ensure that all organisations are given the opportunity to participate in the events. Please let the PMO know if you would like to present at any future events we are invited to. We will give you full support to ensure the presentation is consistent with all Vanguard communications and that you are fully briefed on the subject matter. We would also like to know if you have any requests to present about Vanguard so that we can give you support in delivering the right messages to your target audience, as well as ensure the most appropriate people attend. The PMO would like to welcome Fran White and Rosie Fowler to the team. Fran will be supporting Vanguard as project management officer and Rosie will be helping out with the administration. Sally Banister will be joining the office as programme manager on Monday (July 13). Integrated health and social care teams Five integrated health and social care teams will be up and running in Farnham, Fleet, Farnborough, Aldershot and Yateley by the end of this month. These integrated care teams (ICTs) are made up of community nurses, occupational therapists, physiotherapists, social workers, a psychiatric nurse, a lead psychiatrist, a pharmacist, a geriatrician, GPs, the voluntary sector, and specialists in palliative care and domiciliary care, supported by a dedicated ICT Coordinator. Gethin Hughes, workstream lead, said: “Things are moving fast and we’re making real progress – the teams will soon be meeting regularly, discussing patients and working from a single, coordinated care plan. Through ICTs we can effectively manage the most vulnerable and complex patients in the local community and provide more joined-up care. Increasingly, these clinicians and other care professionals will work as a single team to better serve local patients. “It’s all about supporting and empowering people to maintain independence and control, manage their multiple health and care conditions locally and receive care close to their own homes wherever possible. We’re already seeing some improvements in patient experience.” Each team will have a local central base, known as a hub, where they will meet and work closely together. Confirmed bases are Fleet Hospital, Farnham Hospital, Aldershot Centre for Health and Oaklands Surgery in Yateley. Options for a base in Farnborough are being investigated. The five ICTs will be jointly meeting on 6 August to compare early experiences and share successes and learning so far. A launch event for a wider audience, including all GPs, PPGs, providers and the voluntary sector, is planned for September. The lead GPs and project implementation leads (PIL) are: Fleet: Jenny Bramwell, GP to be confirmed Farnham: Dr Ed Wernick, Dr David Brown and Sophie Baker Yateley: Dr Gareth Robinson and Fran Campbell Aldershot: Dr Lesley Rosling, Dr Usha Couderc and PIL to be confirmed Farnborough: Dr Sharon Boylett and Fran Campbell/ Aiden Lewis/ Fiona Harris Latest updates from workstreams Preventing ill health, enabling self care and supporting wellbeing: This programme focuses on stopping illnesses (both physical and mental health) escalating to a point where professional intervention is needed. The prevention workstream has done substantial work on scoping out the projects the system should focus on to make a real difference in prevention and self care. A compelling business case will now be written to set out what benefits can be delivered. New model of commissioning: A discussion paper has been drafted – Integrated commissioning to support joined-up care, better health, better value. The paper outlines: What will happen if commissioning remains as it is Outcomes specification for the new model of care Options available for integrated commissioning The discussion paper will go to partners later this month. An informal briefing will be given to the CCG's Governing Body and to cabinet members. Plans to involve patients and the public are being developed before presenting a preferred option for consultation in the autumn. Workforce: This active workstream will be pulling together a recruitment campaign to attract more unqualified staff to the workforce. The integrated care team coordinator job advert has been advertised, with interviews scheduled for 23 July. Communications: Generic material is now available online at www.northeasthampshireandfarnhamccg.nhs.uk/vanguard. Material available includes a slide pack for presentations, key messages, latest position statement, social media messages and a generic board paper for partners. More material is being updated daily, so please check it out. This material is designed for all partners to use and can be easily adapted for key audiences. Health and Care Innovative Expo 2015 North East Hampshire and Farnham Vanguard has been invited to the Health and Care Innovative Expo 2015 (Expo 15) to showcase our programme. The theme of Expo 15, the only conference hosted by NHS England, is the Five Year Forward View. Around 10,000 delegates from both health and social care are expected to attend the twoday event on 2-3 September in Manchester. Speakers include: Simon Stevens (Chief Executive, NHS England); Professor Sir Malcolm Grant CBE (Chair, NHS England); Tim Kelsey (National Director for Patients and Information, NHS England); and Mark Britnell (Chairman, KPMG’s Global Practice). This is an excellent opportunity for both Vanguards and the national new care models team to come together for the first time as a community to share our work to date and plans for the future with the wider health and care community and collectively benefit from their support. We have some complimentary entry tickets to this event, as well as exhibition space.
