11.1.5 A.3 COMMUNITY HEALTH CENTER INFECTION/DISEASE CONTROL SPECIALIST JOB DESCRIPTION Description: Under the direction of the NE State department of Health and Human Services System, coordinate infection and disease investigation services for patients at the GNCHC, monitors and track infectious diseases, bioterrorism prevention and control; review clinic infection control policies, health resources, and services to determine impact of disease prevention, control, and response; analyzes available data on disease and quality of care within Good Neighbor Community Health Center. Monitor the clinic for compliance with infection control guidelines, policies and procedures. Will provide assistance anywhere in the state at the request of the Nebraska Department of Health and Human Services, in the event of : 1. A communicable disease outbreak; 2. A potential bioterrorism threat; and or 3. A need for the services of an epidemiologist in investigation and follow-up activities. Duties and Responsibilities: 1. Report communicable diseases and other reportable conditions to the Nebraska Department of Health and Human Services utilizing the Nebraska Electronic Disease Surveillance System on-line reporting mechanism to report to DHHS as specified in the Nebraska administrative code title 173 Communicable Diseases, Chapter 1, Reporting and Control of Communicable Diseases. See Code 2. Create and Maintain disease specific protocols to include reporting and followup procedures for targeted reportable diseases and events. These protocols will be developed and updated over time at minimum every three years. 3. Complete chart surveillance for assessment of quality care. Track all required performance measures for the Health Center. 4. Monitor for trends in targeted communicable and chronic disease incidence. This includes tracking in house acquired infections related to procedures conducted at GNCHC/ECDHD. 5. Coordinate with the ECDHD surveillance staff and Public Health Emergency Response coordinator for procedures for potential bioterrorism and emergency events. Meet with ECDHD staff a minimum of twice per year on these issues. 6. Participate in public health related exercises to include pandemic influenza coordinated by the ECDHD. 7. Monitor and update policies as appropriate for meeting the needs of special populations individuals during a time of emergency. This would include but not be limited to people with disabilities, people with serious mental health illnesses, minority groups, non-English speaking individuals, children and the elderly. 8. Completion of any other contract deliverables as outlined in the Infection and Disease Specialist Contract between the Nebraska Department of Health and Human Services Division of Public Health Office of Preparedness and Emergency and ECDHD and GNCHC 9. When a case of active Tuberculosis is identified follow Nebraska Department of Health and Human Services and CDC protocol to implement surveillance and reporting activities as appropriate including, but not limited to administration of direct observation therapy, Refugee-Immigrant Surveillance & Investigation and all items listed under number 11, below. 10. Management of latent Tuberculosis reporting for GNCHC 11. Responsible to lead Safety Committee including; a. Conducting regularly monthly meetings to receive reports on the standard agenda and monitor for trends and needs for improvement in regards to Joint Commission Requirements b. Compile a report for the Quality Assurance Committee of the GNCHC board and present that report each time the QA Committee meets c. Compile and submit an annual Safety Committee Summary Report to the joint ECDHD/GNCHC boards for each calendar years. Spearhead creation of, annual modification and application of the Safety Committee’s Infection Control Plan 12. When a reportable communicable disease is identified; a. Collaborate with Nebraska Department of Health and Human Services and with the Center for Disease Control and Prevention to implement surveillance activities as appropriate. b. Initiate communicable disease tracking as defined by specific disease causes and modes of transmission in order to get appropriate control and prevention measures. c. Report all possible contacts and investigate accordingly. d. Assure compliance with treatment to safeguard the health of the community. e. Provide guidance for any staff involved in communicable disease support (ie latent TB program) through training and monitoring exercises. 13. Complete all Other Duties as assigned Reports to: Nursing Quality Manager Supervises: Data Entry Clerk assigned to assist in duties. Any ancillary staff assigned to specific communicable disease outbreak or bioterrorism event. Job Description Requirements: Bi-lingual in English and Spanish. Must have cultural competency within the Hispanic/Latino population. Must have a strong medical or nursing background. Performance Knowledge, Abilities and Skills Required: Basic Epidemiology Courses to include CDC home study course, Principles of Epidemiology SS3030 and the eight day Applied Epidemiology course
