ALTERNATIVE FUELS INCENTIVE GRANT
INSTRUCTIONS FOR COMPLETING
INNOVATIVE TECHNOLOGY SCOPE OF WORK
1. List the Project Name exactly as identified in the first “WHEREAS” paragraph of the Grant Agreement.
2. List the name of the Grantee exactly as identified in the first paragraph of the Grant Agreement.
3. List the individual who will be the primary point of contact for the grant, including phone number and e-mail
address. List the individual who will serve as the secondary point of contact, including phone number and e-mail
address.
4. List the street address of the Grantee exactly as identified in the first paragraph of the Grant Agreement. If there
are multiple project locations, check the appropriate box and list all additional project locations on a separate
sheet.
5. Indicate whether or not there are any project partners and identify the name and contribution of any partners if
applicable.
6. Briefly describe the project, including specific grant goals and objectives.
7. List the planned completion dates and critical sub-tasks of all project milestones. Enter the corresponding
percentage of grant spent at each milestone. Do not leave blanks. Enter “NA” if not applicable. Enter “completed”
and date completed if milestone has been completed and enter “0” under corresponding % of grant spent if
milestone related costs are not part of grant.
8. List project deliverables to be tracked (i.e. reports and information that will be sent to DEP that will clearly
demonstrate that an activity occurred). Required deliverables include:
 Monthly Progress Reports (electronic) – due to the DEP project advisor by the 5 of the month following the
end of the quarter being reported.
 Final Report – due to project advisor within 30 days of the completion of the project.
 One-Year Follow-Up Report – due one year after the completion of the project.

Conditional deliverables include:
 Any required permits.
 Technical papers resulting from the project.
 Photographs.
9. Disposition of Equipment – If equipment will be purchased with grant funds, please describe plans for
management of the equipment at project completion. Grantees may not dispose or convert property or equipment
acquired under this grant for purposes other than the original project without the prior written approval of the
Alternate Fuels Incentive Grant Program.
10. Estimated Energy Performance Outcomes – Enter any fuels saved or generated as a result of project deployment.
11. Estimated Investment in Economy – Enter the amount to be spent on goods or services.
12. Economic Development Benefits – Enter the savings to Pennsylvania consumers and the value of contracts with
Pennsylvania companies. Indicate type and amount of energy saving or generating components.
13. Environmental Benefit Data – Enter the quantity of air pollutants to be reduced.
14. Estimated Job Creation and Retention – Enter the number of jobs directly created or retained as a result of project
deployment.
15. Permits – Enter all permits required to complete this project. Enter NA if not applicable.
16. Property Easements – Indicate whether there are any property easements or landowner agreements required, and
if so, if the documents have been provided to DEP and identify the landowner(s).
17. Subcontractors – Indicate whether subcontractors will be used for the project. If yes, specify the name of the
subcontractor and the information requested including the process by which they will be selected (e.g. bid, sole
source, direct award, etc.). If not yet selected, under “subcontractor” indicate “TBD – will provide name and
Federal ID number to DEP for approval prior to start of work”. Describe the process by which subcontractors will
be selected, e.g. competitive bid.
18. Project Budget – Enter the Grant Amount (DEP contribution) the Match Amount, and the Total Project Costs,
which equals the sum of the Grant Amount and Match Amount.
