Established the accuracy and clinical utility of CT angiography (CTA) for the diagnosis and
characterization of acute neurovascular disorders, such as stroke and carotid artery dissection.
Pilot studies demonstrating the feasibility and potential added clinical value of MR perfusion
imaging (MRP) for: (i) brain tumor angiogenesis, staging, and outcome prediction; and (ii) acute
stroke diagnosis and treatment.
Pioneered the use of CTA-source images in patient selection for thrombolytic stroke therapies.
Numerous peer reviewed publications, reviews, edited textbooks, case reports, chapters, and
invited lectures that developed, defined, and communicated the roles of innovative physiological
imaging techniques in the management of diverse, clinically important neurological and head &
neck disorders, such as parotid gland tumors, neurofibromatosis type II, schwannomatosis, atrial
myxoma brain emboli, cerebral schistosomiasis, and inflammatory amyloid angiopathy.
Established the accuracy and clinical utility of CT perfusion imaging (CTP) for the diagnosis
and characterization of cerebral ischemic and neoplastic disorders;
Determined optimal digital image display parameters for acute stroke detection using
unenhanced head CT scanning.
Early efforts at technical optimization and radiation dose reduction for routine head CT scans,
most recently involving novel iterative reconstruction and dual energy scanning techniques.
Introduced and helped to validate the CTA “spot sign” – an important predictor of hematoma
expansion for patients with acute intracranial hemorrhage.
Introduced and championed pilot studies of a promising, novel portable device for the detection
and monitoring of stroke, hemorrhage, and traumatic brain injury - "Electrical Impedance
Spectroscopy” - effectively an "EKG for the brain", developed at the MGH-MRI center.
Introduced a novel CTA imaging marker for vulnerable internal carotid artery plaque.
Successfully challenged the accepted paradigm regarding the optimal metric for determining
admission infarct size using CTP datasets acquired and post-processed with “current generation”
scanner platforms (specifically, recommending the use of “flow” rather than “volume” maps).
Pioneered a novel “atlas-based” toolkit for brain structure-function correlation with CTP/MRI.

Narrative: Clinical Expertise and Innovation Pathway
The unifying theme of my academic achievements has been the advancement and dissemination of novel methods
for imaging brain structure and function in general – and the neurovascular system in particular – not only through
equal parts clinical excellence and translation research, but also through teaching at the local, national, and
international levels. I have been internationally recognized as a leader in developing, defining, and communicating
the roles of innovative physiological imaging techniques in the management of diverse, clinically important
neurological disorders including stroke, cancer, and traumatic brain injury.
My numerous peer reviewed publications, reviews, edited textbooks, chapters, and invited lectures - establishing CT
angiography (CTA) and perfusion (CTP) imaging as convenient, low cost alternatives to MRI - have significantly
influenced the triage of stroke and brain tumor patients worldwide. Indeed, many of my high-impact first and senior
authored papers of the past 14 years – some highlighted by journal editorials and the lay press - have pioneered the
emergency use of CT/CTA/CTP in patient selection for thrombolytic stroke therapies (“Peer reviewed major references” section of
CV, refs 10, 15, 19, 25, 39, 43, 61, 63)
, potentially shifted the accepted paradigm regarding the optimal metric for determining
admission infarct size using CTP (48, 57, 83, 84, 86), and set the agenda for imaging studies of brain tumor angiogenesis,
staging, and outcome prediction (3, 28, 37).
Together with colleagues in other disciplines from the US and abroad, my Neurovascular Lab research group has
been among the first to highlight unique imaging features of a common parotid gland tumor (5), report an important
pitfall in the MRI detection of subarachnoid hemorrhage (9), advance routine 3D post-processing of large CT/MRI
datasets (17, 31, 50, 64), explore the influence of clinical history availability on the accuracy of head CT interpretation

, pilot efforts in head CT radiation dose reduction (29, 87, “edited textbooks” ref 5), characterize CNS schistosomiasis (“Case
reports” ref 8)
, schwannomatosis (34), atrial myxoma emboli (“Case reports” ref 7), and amyloid angiopathy-related inflammation
(42)
, introduce a novel CTA marker for vulnerable carotid plaque (54), describe the CTA “spot sign” – an important
predictor of intracranial hematoma expansion (41, 59, 66), and develop tools for brain structure-function correlation with
CTP/MRI (75, 83, 89). My research has been well funded by government, foundations, and industry; including recent
NIH/DOD support to develop a promising portable device for the detection of brain hemorrhage - "Electrical
Impedance Spectroscopy” - effectively an "EKG for the brain", invented at the MGH-MRI center (74).
These efforts have been enhanced by leadership on many national committees, notably the AHA perfusion imaging
guidelines committee (“National guidelines” ref 2), the AMA Physician Consortium for Performance Improvement (PCPI)
Stroke/Stroke Rehab Workgroup, as an organizer of the NIH Stroke Roadmap consortium (“National guidelines” ref 1A/B), as
a Past-President of the Eastern Neuroradiological Society, and recently on the NINDS Stroke Progress Review
Group (SPRG) for imaging. I am an elected Fellow of both the American Heart Association and the American
College of Radiology. Locally, I have received “Partners in Excellence” and “Nathaniel Bowditch” awards for
service on the MGH Acute Stroke Quality Taskforce (ASQT) and CT Radiation Dose Optimization teams. I served
as Deputy Editor for neuroimaging of the pre-eminent, flagship journal of the Radiological Society of North
America, Radiology, from 2007 through 2009, continuing as Special Consultant Deputy Editor, and have been on
the editorial board of the American Journal of Neuroradiology since 2000. I also serve as an oral examiner for the
American Board of Radiology. Perhaps most importantly - as I hope my CV conveys - I am passionate about
teaching and mentoring at all levels, culminating in my Spring 2011 launching (with my mentor, Dr. Gonzalez) of a
new, very well received Harvard CME course, “Intensive Review of Neuroradiology”.
