The Weslaco Independent School District Employee Health Benefits Plan (the Plan)is pleased to announce a new provider choice for members of the Plan. Beginning December 1, 2016, Plan members will have access to the Texas Free Market Surgery Center in Austin, TX at no cost to the member! This does not change any other benefits of the Plan. The next few pages will tell you about the Texas Free Market Surgery Center. Instructions on how to seek treatment are at the end of this presentation.

Ancillary Expense Reimbursement Policy for Medical Treatment at the Texas
Free Market Surgery Center, Austin Texas

Ancillary expenses will be reimbursed only if a Participant of the Employee Health
Benefits Plan Weslaco Independent School District (the Plan) receives medical
treatment at the Texas Free Market Surgery Center, Austin Texas that is a Covered
Charge under the Plan.
The Plan must be notified before travel expenses are incurred.
Incurred Ancillary Expense Reimbursement Schedule:
Travel expenses to and from Austin limited to $425.
Lodging expenses in Austin limited to $200 per night.
Meals: Breakfast – up to $10, Lunch – up to $15, Dinner – up to $25. Meals for
one parent/guardian accompanying a minor Participant patient will be eligible
for reimbursement.
Any reimbursement will be for actual expenses incurred and paid by the Participant.
Legible receipts must accompany all requests for reimbursement.
The maximum amount (all expenses combined) the Plan will reimburse is $1,000
Travel Reimbursement will be coordinated through Frates Benefit Administrators
