Quality & Environmental Health Laboratory How to use our new Sample Submittal Chain of Custody Form Please complete all fields as best as possible Each (batch of) sample(s) submitted will be given a unique code, please use this code in all queries related to the submitted samples Input the name you want to give your sample Use the sample type code key to indicate the type of sample. E.g. For Marine Water write ‘MW’ List the parameters you wish tested. Use the table on page 2 as a guide. Indicate the number of containers submitted for each sample Each sample submitted will be given a unique code that enables tracking through our lab. Write out any special instructions or considerations you wish to be given to the sample. Please note this information List of Parameters offered by QEHL. This list is just a guide.
