Acceptance and exclusion criteria and thresholds / Referral Guidelines for Optometrists / Mediscan Booking
Procedures for Patients
All children (under the age of 18 years) will be referred directly through to Secondary Care and will be excluded
from this service.
Referral Criteria and Sources
Adults aged 18 or over will be referred into the Service via GPs, Optometrists and Secondary Care clinicians.
The only acceptance criteria that will apply to the Service relates to age (18 years old and above). However, if following
triage and/or assessment, the referral is not appropriate, or the person’s needs cannot be met, the referral will not be
accepted into the Service and the patient directly referred onwards to the appropriate service and the patient’s GP will be
notified of this decision within the timescales set out above.
Access to targeted/specialist services will be determined by use of appropriate assessment.
There are three key elements to this service:

Triage of all new referrals
Paper triage of all referrals with those not appropriate for service directly referred to hospital eye services. Where
paper triage is not sufficient on clinical grounds face to face triage may be required.

Rapid access assessment and treatment (new patients)
Conditions suitable for this referral route would include:
Undiagnosed red eye
Iritis
Posterior Vitreous Detachment – flashers and floaters (In a minority of cases)
Contact Lens related problems (in a minority of cases) Episcleritis

Routine assessment and treatment (new patients)
New routine referrals for assessment and treatment and discharge for a range of conditions where secondary care
intervention is unlikely to be required, including:
Conjuctivitis (not responding to Rx
Periorbital cycts
Conjunctival cysts
Pingueculae and pterygia (only symptomatic or troublesome)
Dry eyes
Concretions
Blepharitis
Epiphoria
Concretions
In growing eyelashes - epilation
Simple Contact Lens related problems
Exclusion Criteria for treatment in the service

The service is not suitable for the treatment of patients where surgery is required for trauma or malignancies. Such
patients should be referred to an appropriate hospital secondary care eye service without delay;
Conditions which are excluded from treatment within the service include:

Sudden loss of vision

Squints

Patient requiring orthoptic assessment

New distortion for macular degeneration needing urgent angiogram

Double vision requiring orthoptic assessment

Malignancies, including skin cancer

Cataract removal

Trabeculectomy

Posterior capsular thickening

Diabetic retinopathy

History of complex ocular pathology

Retinal tear/hole

Wet Age Related Macular Degeneration (AMD)

Glaucoma referral refinement (Currently part of Local Enhanced Services)

Ocular Hypertension monitoring (Currently part of Local Enhanced Services)

MEDISCAN DIAGNOSTIC SERVICES OPHTHALMOLOGY SERVICE
Booking Appointments:
 Mediscan office will be open for booking appointments at 9.00am until 5.00pm –
Monday to Friday.
 Outside of these core hours Mediscan will take messages i.e. 8.00am-9.00am,
5.00pm-7.00pm and Weekends – Saturday [9.00am-5.00pm], Sunday [10.00am4.00pm]
 Patients will be given an Urgent Call back during office hours Monday to Friday and on
Monday morning post weekend.
 All messages taken will be treated as Urgent patient recalls.
