MAPPIM: A guide for referrers Maternity and Perinatal Partnerships in Mental Health THE MAPPIM SERVICE MAPPIM is a multi-disciplinary mental health service for pregnant and postnatal women with severe and complex mental illness, having their maternity care at St Thomas’ Hospital. MAPPIM aims to detect, prevent and treat perinatal mental health problems. Consequently we hope to improve the experience of pregnancy and the postpartum period for women and their families. Women at risk of relapse following delivery are assessed early in pregnancy to allow them time to consider the treatment and support options available. MAPPIM works collaboratively with women, their families and other professionals, to plan care for pregnancy and the early postpartum period. The team also offers preconception advice for women concerned about the implications of mental illness and medication when planning a pregnancy. MAPPIM works jointly with adult and child mental health services already providing care for a woman when she becomes pregnant. MAPPIM also works in partnership with maternity services, children and families social services, primary care, other SLAM mental health services. REFERRAL CRITERIA Any professional can refer a women having maternity care at St Thomas’ Hospital who is pregnant or up to 6 weeks postnatal. Referrals are essential for women with: Bipolar Affective Disorder Schizophrenia / Schizoaffective disorder Previous Postpartum Psychosis Severe Depression Other psychotic illness Family History of Postpartum Psychosis Please discuss any other women who you consider has a severe or complex mental illness and might benefit from psychiatric assessment. Such problems might include Post Traumatic Stress Disorder, Eating Disorders and Obsessive Compulsive Disorder. We also accept referrals for women seeking preconception advice. If substance misuse is the primary problem please refer to the LANDS/Woodvine clinic. HOW TO REFER All referrals are taken by telephone (Tel: 020 7188 3610) from 9am to 5pm Monday to Friday. One of our clinical staff will discuss the referral and ask for information to help us to determine the woman’s needs. If assessment is not considered necessary referrers will be offered advice or directed to other appropriate services. If there is significant concern about a woman who does not wish to be referred she can be discussed anonymously for advice. Out of hours referrals: Urgent referrals for maternity inpatients - contact the duty psychiatrist (bleep 0123). Urgent referrals for women in the community - contact the GP or ask the woman to attend the A&E department. Non-urgent referrals – please leave your name and contact details only and we will phone you. CONTACT DETAILS MAPPIM Adamson Centre St Thomas’ Hospital Lambeth Palace Rd London SE1 7EH Tel: 020 7188 3610 Fax: 020 7188 3611 FURTHER INFORMATION THE MAPPIM TEAM The team includes a team leader (RMN), consultant psychiatrist, specialist registrar in psychiatry, perinatal mental health worker (RMN), and administrator. LIAISON MEETING A weekly multi-agency meeting is held on Thursday mornings in the antenatal clinic at St Thomas’ Hospital. It is regularly attended by Liaison Children and Family Social Workers and midwives. All maternity and mental health staff are welcome to attend to discuss new referrals or women known to MAPPIM. TELEPHONE ADVICE Clinical staff are available from 9am - 5pm Monday to Friday to discuss potential referrals, women known to MAPPIM and also treatment options for women with less severe mental health problems who may not meet the criteria for assessment. ASSESSMENT AND TREATMENT Outpatient appointments are held in the antenatal clinic at St Thomas’ Hospital, and initial assessments are offered within 4 weeks of referral. Women are also seen on the maternity wards, either for assessment or to review their care following delivery. The MAPPIM team is not able to provide community visits.
