JOB DESCRIPTION
POST TITLE: Respiratory Care Nurse
BASE (DEPARTMENT/WARD ETC): Primary and Integrated Care
GRADE/SCALE: Band 6
FULL/PART TIME:
MANAGERIAL ACCOUNTABILITY AND PROFESSIONAL ACCOUNTABILITY (IF
APPROPRIATE):
MAIN PURPOSE OF THE POST:
 To work as part of a countywide team leading the care for those suffering with
chronic respiratory disease.
 To work across primary and secondary care to reduce the need for acute service
intervention – reducing admissions, supporting early discharges, supporting
colleagues and empowering patients to manage their condition with the ultimate aim
of improving quality of care.
PRINCIPLE DUTIES AND RESPONSIBILITIES
Clinical
1. To assess, plan, deliver and evaluate individual programmes of care in a response to
planned and urgent need.
2. To take responsibility and accountability for a designated caseload of patients,
working as their Primary Nurse.
3. To deputise for the Clinical Lead in their absence.
4. In partnership with Pulmonary Rehabilitation Lead and Clinical Lead participate
where appropriate in programmes of Pulmonary Rehabilitation including:
 Delivering aspects of education
 Identifying suitable patients for the programme
 Supporting patients through the programme with a focus on their long term
management
5. To be an expert clinical resource.
6. To provide expert clinical education and support to: Medical staff of all levels in primary and secondary care
 Nursing and therapy staff of all levels in primary and secondary care
 Patients and their carers (formal and informal)
 Students
7. To contribute to palliative care of chronic respiratory patients.
8. To develop partnership working with all relevant agencies/service/individuals to
support optimum management of the identified client group.
9. To undertake planned specialised procedures where necessary – in line with service
developments.

11. To be competent to work across primary and secondary care supporting the
management of patients in both acute and chronic phases of their condition.
12. To contribute to the development of specialist protocols and care pathways.
13. To maintain accurate documentation and records in line with Trust policy and NMC
code of conduct.
Management
 To ensure service is developed as a needs led service with consideration being given
to demographics, partner service developments.
 To participate in the organisations annual appraisal process.
 To lead the development of links with case managers and their role in supporting
chronic respiratory patients.
 To understand/be aware of the financial position of the service.
 To recognise workload pressures for self and others, seeking appropriate support.
 To contribute to qualitative and quantitative audit of service and ensure accurate
information is maintained both at service and organisational levels.
 To co-ordinate all aspects of the service in the absence of the Clinical Lead.
Education
 To work with Clinical Lead in devising and delivering formal specialist educational
programmes across primary and secondary care, and partner agencies including
voluntary organisations.
 To work with Clinical Lead to develop team/service objectives.
 To actively foster an environment conducive to the learning needs of staff and
students.
 To provide mentorship for pre, post and specialist students and new staff members
from a variety of professional backgrounds and organisations.
