Pharmacists manage drug therapy of chronically ill and complex
patients through delegated prescriptive authority and collaboration
with other healthcare providers in these contemporary ambulatory
clinics. Reimbursement for cognitive services is collected for many

Clinical Pharmacist, Interdisciplinary Geriatrics Clinic
In this innovative practice model, clinicians from medicine, social
work and pharmacy practice collaboratively, sharing responsibility
for decision-making and longitudinal follow-up. An extension of a
research grant funded by the Robert Wood Johnson Foundation,
this clinical model operationalized a teaching model for medical
trainees within the limitations of current organizational and
payment structures.
2001-2009
Clinical Pharmacist, Turner Geriatrics Clinic
In this well-established ambulatory clinic, consultative pharmacy
services were provided to roughly 80 patients per month, 11
months per year. Following thorough review of each patient,
including review of the electronic medical record and interview with
the patient and/or caregivers, drug therapy plans were designed
and implemented in collaboration with a multi-disciplinary team.
Additional services, including telephonic contacts, home visits and
longitudinal follow-up, were provided as indicated.
1999-2001
Clinical Pharmacist, Airways Clinic, Henry Ford Health System
Nearly 1000 adult patients with asthma were managed in a
disease management model in this new pharmacist-managed,
physician-directed clinic. Patients at highest risk for poor
outcomes were evaluated in clinic; after the diagnosis was
established and treatment initiated, the pharmacist, with delegated
prescriptive authority, conducted follow-up. As many as 6 patients
were seen in a 4-hour weekly clinic, and an additional 5-15
patients per week were contacted for telephonic follow-up. After
one year, improvements in total cost of care and quality of life were
noted among the high risk, intensive intervention group.
1998-2001
Clinical Pharmacy Specialist, Henry Ford Health System
In this managed care environment, the primary responsibility was
to promote rational drug use among nearly 80 providers who cared
for 50,000 patients enrolled in the managed care plan. Strategies
employed included provider profiling with individual and group
education, drug use policy development and implementation, and
disease management.
1996-1998
Clinical Pharmacist – Internal Medicine, St. Joseph Mercy Hospital
Direct patient care was provided year-round for up to 10 medical
intensive care patients per day. Responsibilities included patient
assessment, collaboration during multi-discliplinary rounds, patient
and family education, medical team education, interfacing with the
drug distribution system and documentation of assessments and
recommendations in the medical record. In addition, a pharmacistmanaged anti-coagulation clinic was established, including
preparation of a business plan, development of treatment
algorithms and monitoring forms, selection of a point-of-service

monitoring device, and identification of mechanisms of
communication to patients’ other care providers.
1995-1996
Clinical Pharmacist – Asthma Care Center, Detroit Medical Center
This clinic was developed to address the unique needs of an
under-served population. Pharmacist responsibilities included
recruitment, patient assessment, design of individualized drug
therapy plans, patient education and follow-up. The clinic
operated ½ day per week year-round, and could accommodate up
to 4 patients per week. Low literacy patient education materials
were developed to promote self-management strategies among
this population. Follow-up was conducted telephonically to
minimize the impact of transportation difficulties on patient
retention.
1992-1996
Clinical Pharmacist – Internal Medicine, Detroit Receiving Hospital
New clinical pharmacy roles were established for inpatient adult
internal medicine services, which included individualized
assessment of drug therapy, attendance at patient care rounds,
patient and staff education, implementation of drug use policies,
interfacing with the drug distribution system, and documentation in
the medical record. Four to twenty patients were cared for daily for
11 months per year.
