Right Care Programme in West Essex Presentation to the West Essex CCG Board July 2016 Presented by Dr Angus Henderson and Chloe Atkinson What is Right Care? 1. Helps health economies find where they have low value healthcare. 2. Helps them replace that with high value (optimal) healthcare. • All CCG’s will be required to deliver service transformation through this methodology from April 2017. Value We need as much as high value care and as little low value care as possible. OBJECTIVE - Maximise Value (individual & population) West Essex Right Care Programme • In March 2016 the West Essex Professional Leaders Group agreed the priorities for 2016/2017 as Respiratory, Musculoskeletal and Cardiovascular. • Respiratory began in April 2016. • Musculoskeletal began in June 2016. • Cardiovascular due to begin in September 2016. West Essex Respiratory Programme Commissioning for Data Pack (available online) showed areas to improve outcome and value within Respiratory care as: • Smoking Cessation • Medicine Optimisation • Early Identification and diagnosis • Support following diagnosis • Crisis Prevention • Crisis Management • Length of Hospital Stay for patients Who is involved? • West Essex Core Right Care Team – Data analysis, project oversight • Respiratory Expert Advisory Group – Clinical experts across West Essex system, oversight of programme • Breathe Easy – Form part of Expert Advisory Group, provide patient and carer experience • Design Phase – Front line staff, patients, managers coming together to design different ways of working. Designing the Optimum Pathway On the 2nd and 9th June over 60 front line staff, patients and managers cam together to design optimum pathways of care in the chosen areas. Next Steps for Respiratory Programme • Proposed changes between now and April 2017 include: – Introduction of a Care Plan for patients to hold and accessible by all clinicians – All patients stratified by severity of disease to ensure appropriate support is identified and accessed – West Essex system team overseeing all patients with medium and high severity ratings – Out of Hours advice and guidance line for patients and clinicians Next Steps for Musculoskeletal Programme The MSK Expert Oversight Group and one optimal design workshop have agreed the focus of this West Essex MSK programme as: Common themes around prevention, wider support options, shared decision making, medicines optimisation and support following discharge. Main areas: o Back Pain o Rheumatoid Arthritis o Osteoarthritis o Osteoporosis Next Steps for Cardiovascular Programme • Initial data analysis has shown the majority of opportunity within Coronary Heart Disease. • CHD Optimum Pathway Design Workshops scheduled for the 8th and 22nd September 2016.
