ADHD In The Arab World Ahmad Almai, M.D. Head of Child and Adolescent Psychiatry SKMC, Abu Dhabi Disclosure • I have been a speaker and received honorarium or conference sponsorship from the following pharmaceutical companies: -Eli Lilly -Pfizer -Johnson-Johnson -Bristol Myers objectives • Historical Background and Dx Criteria changes • Review cultural perspectives In Context of Genetic Basis • Review regional and local studies and publications of ADHD • GCMBHS/SKMC ACTIVITIES PUBLICATIONS ADHD IN ARABIAN GULF MEDIA • الجمعـة 17ذو القعـدة 1432هـ 14اكتوبر 2011العدد 12008 • السبت 07:28 2011-09-10مساء • • • • • اضطراب «فرط الحركة وتشتت االنتباه» يصيب %15من أطفال السعودية العالج الطبي أثبت نجاح %90من الحاالت • • غذاء أكثر توازنا لمعالجة فرط الحركة لدى األطفال أشارت دراسة نشرتها مجلة "بيدياتريكس" األميركية االثنين إلى أن غذاء أكثر توازنا قد يفيد األطفال مفرطي الحركة الذين يعانون قصورا في االنتباه وفرط الحركة ،في حال فشلت العقاقير أو العالجات األخرى في حل هذه المشكلة. وبحسب العلماء الذين راجعوا الدراسات األخيرة حول هذا الموضوع ،يصعب تقييم منافع الحميات القائمة على المكمالت الغذائية أو التي تستثني بعض المنتجات الغذائية .وقد يكون مفعول هذه األنظمة الغذائية "وهميا" ليس إال. وبالتالي ،ينبغي اعتبار العالجات الغذائية المتعلقة بقصور االنتباه وفرط الحركة عالجات بديلة، بحسب الدراسة التي أعدها أطباء من كلية الطب في شيكاغو. وكتب األطباء في الدراسة أن "التركيز بشكل أكبر على حث األهل واألطفال على اعتماد نظام غذائي سليم وتجنب األغذية التي تعزز قصور االنتباه وفرط الحركة قد • • • • • • • • • • • • • • • • • • • • • يمكن لألسر التعرف على إصابة أبنائها بفرط الحركة بمالحظة شدة الحركة واالندفاعية وتشتت االنتباه («الشرق األوسط») جدة :أسماء الغابري تختلف نسبة نشاط األطفال وتركيزهم واتباعهم للتعليمات من طفل آلخر ،وعلى الرغم من هذا التفاوت الطبيعي ،فإنه توجد نسبة من األطفال يصعب تواصلها مع المجتمع وتكوين أصدقاء ،إلى جانب النشاط الحركي الشديد الذي يسبب اإلنهاك من قبل المحيطين له ،واللجوء معه إلى أساليب قد تصل للعنف أحيانا للحد من هذا النشاط ،وإرغامه على اتباع التعليمات.إلى ذلك ،شخص أطباء في علم النفس والمخ واألعصاب هذا السلوك المضطرب عند هذه الفئة من األطفال إلى اضطراب عصبي يسمى «اضطراب فرط الحركة وتشتت االنتباه» الذي يعد اضطرابا عصبيا بيولوجيا يصيب األطفال. وتشخيص اضطراب فرط الحركة وتشتت االنتباه يتشابه مع أنواع أخرى من االضطرابات العصبية ،لذ يصعب على الكثير من األطباء تشخيصه ،عدا األطباء ذوي الخبرة والمطلعين باستمرار على األحداث الطبية والمتميزين. ويرى االختصاصيون أنه يمكن لألسرة أن تكتشف إصابة أحد أبنائها بهذا االضطراب من خالل التركيز على مدى شدة وحدة األعراض ،والفترات والظروف التي يظهر فيها السلوك غير المناسب ،الذي يظهر في أعراض ثالثة (فرط الحركة -االندفاعية تشتت االنتباه) وال تكون ظاهرة فقط على الطفل وإنما مهيمنة علىسلوكه ،لدرجة أنها تؤثر سلبا على حياته من النواحي العلمية والسلوكية واالجتماعية .وأكدت الدراسات أن نسبة اإلصابة في السعودية تقدر بـ 15في المائة ،فيما بينت آخر دراستين عن االضطراب؛ األولى في المنطقة الشرقية ،أن متوسط اإلصابة في الدمام يقدر بنسبة 16.7في المائة ،واألخرى في الرياض، تقدر بنسبة 12.6في المائة .وعن كيفية تشخيص المرض ،توضح الدكتورة سعاد يماني استشارية المخ واألعصاب و رئيسة مجموعة دعم اضطراب فرط الحركة وتشتت االنتباه في مستشفى الملك فيصل التخصصي ومركز األبحاث في الرياض، أنه ليس هناك اختبارات مخبرية أو أشعة لتشخيص اضطراب فرط الحركة وتشتت االنتباه ،مشيرة إلى أنه ال بد أن يتم التشخيص من قبل مختص قادر على القيام بعملية التشخيص والعالج كطبيب األعصاب أو النفسية أو اضطرابات النمو. “To see a single daily dose of benzedrine produce a greater improvement in school performance than the combined efforts of a capable staff working in a most favorable setting, would have been all but demoralizing to the teachers, had not the improvement been so gratifying from a practical viewpoint” Bradley C.
