Cavity Risk Assessment
Tooth decay (often referred to as cavities) is the single most common, chronic lifelong disease. In fact, it’s five
times more common than asthma and seven times more common than hay fever.
Many adults have untreated cavities. For example, statistics show 27% of those 20-39 yrs old, 21% of those 40-59
and 19% of those 60 yrs and older have untreated cavities.
This risk assessment tool is designed to help you identify factors that might increase your risk of cavities. Please
note that this tool serves to provide a guideline as to your risk of cavities. It is important to visit your dentist on
a regular basis and discuss your oral health.
How often do you visit your dentist?
At least 2 times/year = 0 points
Once/year = 2 points
Rarely or never = 5 points
Regular dental visits allow the dentist to help prevent and/or treat tooth decay at an earlier stage.
How often do you brush your teeth each day?
2-3 times = -1 (minus one) point 1 time = 2 points
Less than 1 time = 7 points
Frequent tooth brushing is an important part of protecting your teeth from cavities. The American Dental Association suggests brushing your
teeth twice a day with fluoride toothpaste.
How often do you floss between your teeth?
Every day = -1 (minus one) point 3-4 times/week = 2 points
Rarely or never = 5 points
Use of dental floss or other special types of cleaners between your teeth also helps to prevent tooth decay. It is recommended that you floss at
least once a day.
Do you use Fluoride or Xylitol toothpaste or chew gum, mints or other products that contain Xylitol?
Yes = -1 (minus one) pointNo = 4 points
Studies have shown using products with Fluoride or Xylitol (a natural sweetener) helps reduce the risk of cavities.
Have you had cavities in the past?
No = 0 points
Yes = 3 points
Patients who have had cavities in the past are more likely to have them in the future.
How many times daily do you eat sugary foods (such as candy, soda, cookies or juice) between meals?
Only with meals = 0 points
1-2 times = 2 points
3 or more times = 8 points
Sugary foods and drinks increase acid production by bacteria that causes cavities. That doesn’t mean that you can never enjoy these types of
foods; but you should limit sugary foods and drinks between meals to reduce additional acid exposure to your teeth.
Do you wear braces, orthodontic appliances, permanent bridges or partial dentures?
No = 0 points
Yes = 5 points
Wearing braces, appliances, bridges and/or partial dentures may trap plaque on your teeth and also makes it difficult to brush and floss which
increases your risk for tooth decay.
Have you had Preventative Resins placed on your teeth?
Yes = -2 (minus two) points
No = 0 points
Dental Preventive Resins are placed on teeth with deep grooves and pits to prevent decay. Consult with your dentist to see if Preventive Resins
are appropriate for you and your family.
