Quick Reference
Though dental practitioners are not required to include a diagnosis code
when submitting claims to Vermont Medicaid, if they choose to include
codes, they must be valid. For claims with dates of service prior to
September 30, 2015, a valid ICD-9 code should be used and for claims with
dates of service on or after October 1, 2015, a valid ICD-10 should be used.
This document includes a sample of common dental ICD-10 diagnosis
codes, code descriptions and some ICD-10 resources to help you
understand code selections and usage for both electronic and paper
submissions.
The use of appropriate diagnosis codes is the sole responsibility of the
dental provider. All links to third-party websites are listed for convenience
only and are not endorsed by Vermont Medicaid.
