ICD-10 Dental Diagnosis Codes Quick Reference Though dental practitioners are not required to include a diagnosis code when submitting claims to Vermont Medicaid, if they choose to include codes, they must be valid. For claims with dates of service prior to September 30, 2015, a valid ICD-9 code should be used and for claims with dates of service on or after October 1, 2015, a valid ICD-10 should be used. This document includes a sample of common dental ICD-10 diagnosis codes, code descriptions and some ICD-10 resources to help you understand code selections and usage for both electronic and paper submissions.
