CONSULTANT IN RESTORATIVE DENTISTRY
Summary
This post will compliment our team of 2.0 Consultant Maxillofacial Surgeons and 2.0
Consultant Orthodontists within NHS Ayrshire and Arran.
NHS Ayrshire and Arran comprises two principle District General Hospitals; Crosshouse
Hospital in Kilmarnock and The Ayr Hospital. The Consultant in Restorative Dentistry
will be an independent part of the Head and Neck Directorate, with maxillofacial,
orthodontic and ENT services. The directorate is part of the Planned Care team within
NHS Ayrshire and Arran. The post will share the facilities of the Maxillofacial and
Orthodontic Unit at Crosshouse Hospital, the main base for maxillofacial and
orthodontic services, and where inpatient, day case and outpatient treatment is
provided.
Crosshouse Hospital (539 beds - base population 225,000) was opened in 1982 and
has been expanded and upgraded regularly since then. It stands on a greenfield site
just outside Kilmarnock and has excellent trunk road access and free car parking.
There is also easy access to rail and air services. It provides local services in all major
specialities as well as area services for Ayrshire in Laboratory Medicine, Renal
Medicine, Infectious Diseases, Dermatology, ENT (including the Scottish National
Cochlear Implant Centre), Maxillofacial Surgery (with laboratory), Orthodontics,
Gynaecology and the South West Scotland Breast Screening Unit. Maternity services
also moved into a new-build unit on the Crosshouse site in August 2006.

The Scope of the Service
An oral and maxillofacial surgery service and an orthodontic service have been
provided in Ayrshire and Arran since 1974 and 1991 respectively. Both services have
two consultants. This post was initially introduced in NHS Ayrshire and Arran in 2005
with the purpose of providing a Restorative Dentistry Consultant service for
rehabilitation of patients with acquired or developmental maxillofacial and dental
defects.
NHS Ayrshire and Arran is responsible for providing an area wide oral health service for
more than 368,000 people living within the Ayrshire and Arran area covering the
following local authority areas.

Regional discussions are ongoing in relation to the possible centralisation of some inpatient service from Crosshouse Hospital to the Southern General Hospital. However, it
is envisaged that only the inpatient service will move and the majority of out patient and
day case treatments carried out in Ayrshire will continue to be carried out in the day
surgery unit
The Maxillofacial and Orthodontic Unit at Crosshouse Hospital has been expanded and
contains 6 dental surgeries and two consulting rooms. The new department was
opened in 2001. Ayr Hospital has outpatient services and facilities for outpatient
treatments, though Restorative sessions are not planned at The Ayr Hospital at present.
Both hospitals have Accident and Emergency departments which are due for further
expansion over the next few years as part of the NHS Ayrshire and Arran Building for
Better Care project.

The scope of the service with reference to Restorative dentistry within Ayrshire is: Maxillofacial Services:
1
Head and Neck Oncology and Reconstructive Surgery.
The department has undergone sub-specialisation. Mr Hislop is responsible for
head and neck oncology and reconstruction service. Mr Dempster & Mr Murray
provide the Otolaryngology input. There is cross referral and joint operating.
The Restorative Consultant would be expected to take part in Head and Neck
Clinics on a monthly basis and provide Specialist Restorative Dental input into
the multidisciplinary management of these patients. Implantology is available for
the rehabilitation of both oncology patients and those with congenital or acquired
facial deformities.

Orthognathic surgery.
A multi disciplinary team treats patients with facial deformity, both acquired and
congenital. The contribution of a Restorative Consultant would be an advantage
to both surgeons and orthodontists.

Trauma.
Hard tissue trauma of the face, and jaws may require the occasional input of a
Restorative Consultant

Cleft Lip and Palate
A Cleft surgeon undertakes the primary repair at Yorkhill Children’s Hospital,
Glasgow. There is however a clinic held in Ayr and there is the opportunity for
future secondary and tertiary work including orthognathic surgery and oral
rehabilitation to be carried out in Ayrshire.

Dento alveolar surgery.
This constitutes a major part of the work carried out by the unit, which is mainly
dealt with by the specialty doctors and surgical trainees under Consultant
supervision.
Occasionally the Restorative Consultant may be required to give advice in
relation to proposed treatment plans.
Orthodontic Services:
1.
Hypodontia
A number of orthodontic patients undergoing routine orthodontic treatment
require advice / treatment for small or missing teeth. Some patients have more
extensive hypodontia requiring long term planning for implants
2.
Cleft Lip and Palate
Orthodontic treatment is provided for these patients within the National Managed
Clinical Network for Cleft Lip and Palate (CLEFTSiS) and may require post
orthodontic restorative treatment.

Facilities Supporting Maxillofacial and Orthodontic Services at Crosshouse
Hospital
The Maxillofacial and Orthodontic teams share a suite for out-patient work at
Crosshouse Hospital. There are also office accommodation and junior staff rooms in
the unit. Nursing support is available to the Restorative Consultant. The Consultant
would have office accommodation and access to a medical secretary. There are two
secretaries for the Maxillofacial Unit along with two part-time typists. The Orthodontic
unit also has two secretaries.
Crosshouse also has a full supporting diagnostic facility including a spiral CT scanner
with 3D reconstruction and a new MRI scanner. There are also new CT and MRI
scanners at Ayr Hospital.
There is a fully equipped oral and facial laboratory staffed by five technicians, one of
who has a prosthetic training and two of who have training in Oral and Maxillofacial
Prosthetics. Both intraoral and craniofacial prostheses are made at Crosshouse
Hospital.
Education:
The organisation is very fortunate to have the Alexander Fleming Education Centre.
This is conveniently sited in the centre of the Hospital, and has a well stocked library
with good electronic facilities, a lecture theatre with state-of-the-art equipment and
meeting rooms. These facilities are currently being extended.
The Directorate is fully involved in the teaching of Glasgow University undergraduates
and the postholder will have the opportunity to contribute to both undergraduate and
postgraduate teaching programmes.

The Post
Consultant in Restorative Dentistry for NHS Ayrshire & Arran based primarily at
Crosshouse Hospital.
2.
Proposed Weekly Timetable:
Each Consultant will require to agree a job plan with the Associate Medical
Director/Clinical Director. This will be reviewed on an annual basis or more
frequently and revised as necessary to meet local, regional and national clinical
priorities.
Through the West of Scotland Regional Planning Group a programme of work is
underway to redesign Restorative Dentistry provision across the West of
Scotland and NHS Ayrshire & Arran is participating fully in this work.
Implementation of the redesign is likely to impact on this post. It is expected that
the postholder’s activities and the location of these activities may change with
greater contribution to regional working. Any change would be taken forward
through consultation and job planning.
The Consultant in Restorative Dentistry will have the following weekly

an orthodontic clinic on an ad hoc basis. Also ad hoc theatre attendance may be required.

3.
Duties and Responsibilities
The main duties and responsibilities of the post include:

i.
Provision of restorative dentistry service including specialist treatment for
patients requiring Maxillofacial oncology rehabilitation and for those with
craniofacial and developmental problems within the context of the
Multidisciplinary Team.
ii.
The postholder will also be responsible for planning rehabilitation for
patients undergoing major head and neck surgery and those with
developmental problems, which may be treated by both Surgical and
Orthodontic colleagues. This will be facilitated through the monthly Head
and Neck clinics and monthly Implant clinics. Combined orthodontic
clinics will be held as required. The postholder will provide the advanced
restorative treatment required by these patients.
iii.
Contribute to the professional supervision of Junior Medical Staff.
iv.
Participate in the development of a restorative services network in NHS
Ayrshire and Arran.
v.
Responsibilities for carrying out teaching, accreditation and examination
duties as required, and for contributing to undergraduate and
postgraduate medical education. The postholder will be expected to
comply with College recommendations on Continuing Medical Education.
vi.
The postholder will be required to comply with NHS Ayrshire and Arran
Policies on Clinical Governance.
vii.
Responsibilities which relate to a special interest where agreed by NHS
Ayrshire and Arran
viii.
Requirements to participate in medical audit and in continuing medical
education.
ix.
Involvement in research where approved by NHS Ayrshire and Arran.
ix.
Work on behalf of NHS Ayrshire and Arran, such as domiciliary
consultations or services provided for other agencies, where appropriate
and agreed.
General Provisions
The post is within the Head and Neck Directorate and the postholder will be
accountable to the Clinical Director who will agree the Job Plan.
He/she will be expected to work with local managers and professional
colleagues in the efficient running of services and will share with Consultant
colleagues in the medical contribution to management. Subject to the provisions
of the Terms and Conditions of Service, he/she is expected to observe NHS

Ayrshire and Arran’s agreed policies and procedures, drawn up in consultation
with the profession on clinical matters, and to follow the standing orders and
financial instructions of NHS Ayrshire and Arran.
In particular, where he/she formally manages employees of NHS Ayrshire and
Arran, the postholder will be expected to follow the Local and National
Employment and Personnel Policies and Procedures.
He/she will be expected to make sure that there are adequate arrangements for
hospital staff involved in the care of patients to be able to make contact with the
postholder when necessary.

5.
Health and Safety
The postholder is required to comply with NHS Ayrshire and Arran Health and
Safety Policies.
6.
Resources
The medical staff resources of the Head and Neck Directorate are listed
elsewhere. Medical staff in the Head and Neck Directorate are supported by a
wide multidiscplinary team including ward and outpatient nursing staff, dental
nurses, a part time dental hygienist, a team of maxillofacial prosthetists among
other general hospital staff.
The Head and Neck inpatient services are provided from one inpatient ward with
29 beds. In addition there is access to a 5 bedded Intensive Care Unit.
The postholder will have access to such general administrative support as is
required for the discharge of his/her duties and responsibilities.
This will include the provision of adequate secretarial and clerical support and
the availability of accommodation, equipment etc.
The postholder will receive support from such other professional staff as are
employed within NHS Ayrshire and Arran and are deployed to his/her area of
patient care.
7.
Annual Appraisal
The successful candidate will also be required to participate in annual appraisal.
Objectives agreed following the appraisal process will be discussed and taken
account of during the annual job plan review.
8.
Terms and Conditions of Service:
Are those determined by the Terms and Conditions of the New Consultant Grade
(Scotland) as amended from time to time. The distance that a consultant can
reside from the hospital, where travel time is seen as more important than
mileage, is subject to the agreement of the Associate Medical Director.
