Dr. Example One [Street Address] [City], [Postal Code] Phone: [Your Phone] Fax: [Your Fax] E-Mail: [email protected] Web: [Web Address] Date: Dr. Example Two Cornea Specialist Ophthalmology Group [Address 1] [Address 2] Dear Dr. Example Two, I graduated from Nova Southeastern University with my Doctor of Optometry degree in 2001 and completed a Contact Lens & Cornea Residency in 2002 at University of Houston College of Optometry. During my residency I had the opportunity to gain further specialty contact lens fitting experience. Over the past 13 years, I have established my Optometric practice in the Tampa bay area. I have successfully been fitting many of my patients with scleral lenses and I am excited to inform you about this service that can be very beneficial to your patients. Scleral lenses have become a gold standard in successfully managing patients with corneal irregularity secondary to keratoconus, corneal transplant, corneal surgery, ocular trauma and ocular surface disease (OSD). Scleral lenses are especially beneficial for patients with corneal irregularity because they vault over the entire corneal surface and land on the sclera. Scleral lenses improve vision by providing a clear optical lens that masks the distorted areas of the cornea creating a smooth, regular optical surface over the patient’s very irregular cornea. Scleral lenses are comfortable to wear because they rest on the less sensitive scleral tissue and are fit not to move. For OSD patients, in addition to improving surface quality, a fluid reservoir is created between the lens and corneal surface, which provides continuous hydration throughout the day providing the patient significant relief. Scleral lenses have successfully been used to manage conditions associated with severe dry eye (Stevens Johnson Syndrome, Graft Versus Host Disease, exposure keratopathy). I have included some case studies and articles that I thought you might find interesting. If you have any patients that you think would benefit from this service, I would very much appreciate your consideration in referring the patient to my practice to be fit with scleral lenses. I guarantee that your patients will receive the best possible care and be treated with the utmost professionalism while they are at my practice. For any patients you refer to our practice, we will not treat them for anything else besides the scleral contact lens service and will send them back to you once that is complete. Sincerely, Dr. Example One 1325 Progress Drive | Front Royal, VA 22630 | 877-533-1509
