This form should be completed and submitted to the college that houses the major you are declaring in or changing to. Please make special note of the following:



Traditional students may NOT declare or change to an Accelerated major. Students wanting to do this must contact the Evening Admissions office.
Accelerated (ACCEL) students wanting to change to a traditional major must speak with Financial Aid planning prior to requesting the change, as there are financial implications to this change.
You are responsible for understanding and meeting all degree requirements in your new major/minor based on the academic catalog in place at the time of the change.
