Preclinical course of Endodontics Course program for III year students, 6th term Department of Conservative Dentistry and Periodontology Rationale for course and goals 1. Application of theory and technique in simulation to achieve competency for clinical practice. 2. Demonstrate implication of internal tooth morphology to endodontic therapy. 3. Demonstrate application of rubber dam and techniques of tooth preparation for endodontic treatment. 4. Demonstrate a precision needed in pulp cavity negotiation, preparation and shaping in particular groups of teeth ÷ entrance and pulp chamber preparation in particular groups of teeth (upper and lower incisors, upper and lower premolars, upper and lower molars) ÷ exploration of canal orifices in prepared teeth ÷ negotiation and preparation of radicular pulp cavity employing ‘step back’ technique 5. Demonstrate obturation of the radicular space with lateral condensation of gutta percha. 6. Describe properties, handling, application of medications and materials used during endodontic therapy. 7. Identify, select and demonstrate management of instruments for operative endodontic procedures. 8. Organization of the course Each student will work on a set of 4 extracted human teeth placed in plaster blocks which include 1 incisor, 1 premolar, 1 upper molar, 1 lower molar. Four additional teeth will be selected for the access preparation. Each session will begin with a short seminar discussing issues and practical exercises scheduled for that session and will be followed by students working in simulation according to the daily checklist. Students should stay focused during seminars and listen carefully to instructor’s requests during practice period. Students are expected to acquire the new material from each session’s seminar and their knowledge will be verified during the following course day (a short quiz). Attendance in all scheduled sessions is required in order to pass the course. If a student is not able to attend a course session he/she must inform the course coordinator and the Dean’s office of the intended absence and reason for that. For unexpected absences please contact us as soon as possible or have someone to contact us on your behalf. Theory of each session missed must be acquired by a student and will be verified by the instructor and the practical exercises included in the session done in the period pointed by the instructor. Student will be asked to repeat the whole course if: will not pass the final test 3 times, will not attend three session for unexcused or excused reason, will not make the progress. Assessment of students The criteria for student’s assessment in preclinical endodontic course are as follows: ÷ Attendance (present, absent: excused, unexcused) ÷ Preparation & participation (prepared for the session, procedures started and completed on time, procedures completed as planned) o endodontic cavity preparation (access) in 8 teeth o root canals of 4 teeth preparation with ‘step-back’ technique - performed during sessions o obturation of radicular space in 3 canals of 2 prepared teeth (1 incisor/premolar, 1 molar) employing lateral condensation of gutta percha technique - performed during sessions o interim dressing placement (calcium hydroxide) in 3 root canals using three different techniques ÷ Professional behavior in simulation (properly attired, displayed correct simulated patient/operator positioning, the simulation work place well organized, clean, and uncluttered) ÷ Communication (appropriate use of the instructor’s time, consideration toward fellow students and dental personnel, a positive and receptive attitude to evaluation and feedback) ÷ Written test passed (60%-minimum to pass) ÷ Practical skills evaluation competency passed Course Schedule and Location: 15 sessions (3h each; 4x45min) theory and practice for details please check the Dean’s office COURSE OUTLINE SESSION I THEORY: Introduction to endodontic therapy Morphology of endodontium, principles of endodontic pathology Rubber dam as the necessary isolation tool in endodontics PRACTICE: Rubber dam placement using different techniques SESSION II quiz (session I) PRACTICE: short presentation on teeth selection for root canal treatment (RCT) selection, assessment, measurements of extracted human teeth preparation of plaster blocks for a simulation of RCT SESSION III THEORY: tooth preparation for RCT principles and guidelines of endodontic access preparation – tools and clinical steps pulpotomy, pulpectomy common problems and mishaps of endodontic access preparation techniques of orifices localization endodontic instruments SESSION IV quiz (session III) PRACTICE: access preparation of 4 selected teeth SESSION V PRACTICE: access preparation of 4 more selected teeth (8 in total) orifices preenlargement SESSION VI THEORY: principles, objectives and methods of root canal preparation working length (WL) measurements (electronic apex locators, radiological methods) chemical supply (irrigation and intracanal medications) SESSION VII quiz (session VI) PRACTICE: assessing of trial lengths for each root canal of 4 teeth preparation of teeth for x-ray (radiologic method of WL measurement according Ingle’s technique) SESSION VIII PRACTICE: WL measurements according to Ingle’s method (taking x-rays and values assessment) SESSION IX PRACTICE: practical presentation of the root canal preparation in the endodontic training block root canal preparation with ‘step back’ technique of the endodontic training block root canal preparation with ‘step back’ technique of 4 teeth SESSION X PRACTICE: root canal preparation - continuation SESSION XI THEORY: objectives and principles of root canal obturation methods of obturating the root canal space materials used for root canal obturation teeth restoration after RCT PRACTICE: calcium hydroxide placement as the intracanal medication in 3 root canals with 3 different techniques SESSION XII quiz (session XI) PRATICE: presentation of root canal obturation with lateral condensation of gutta percha of endodontic training block root canal obturation with lateral condensation of gutta percha of endodontic training block root canal obturation with lateral condensation of gutta percha of 3 root canals temporary restoration of endodontically treated teeth SESSION XIII continuation of practical work assessment of practical work (8 teeth) SESSION XIV written final test practical skills evaluation competency (preparation of the root canal in the endodontic training block) SESSION XV practical skills evaluation competency (OSCE stations) filling out the students charts Textbooks: 1.
