HOW TO USE THIS FORM
This form is for students who have been denied student financial assistance as a result of failing the
required credit check and wish to have the decision reviewed. It is important that you carefully read all
instructions.
The OSAP credit check review applies to you if you can demonstrate:
• The circumstances that led to your financial difficulties were beyond your control; and
• You are now financially stable, and will be able to repay your student loans.
Your Credit Check Review Submission
You must complete the form attached to these instructions. You must also provide photocopies of relevant
supporting documentation to prove the claims made in your submission. The documentation you include must
provide information about the specific circumstances that led to your credit check failure and must also include
your loan repayment plans.
Your entire submission must not exceed 20 single-sided pages (or 10 double-sided pages).∗
Remember:
• Submissions that are not supported by relevant documentation are less likely to succeed.
• Only submit:
o documents that demonstrate the circumstances that led to your financial difficulty were beyond
your control.
o documents that demonstrate your repayment plans
Examples of supporting documentation to demonstrate that the circumstances that led to your financial difficulty
were beyond your control may include (but are not limited to):
• letters from a physician or other health professional who is regulated under the Regulated Health Professional
Act, 1991 • death certificates • police reports • letters from lawyers/employers/third parties • proof of job search
history • record of employment (ROE) • layoff/dismissal letters • business licences, etc.
Examples of required supporting documentation to demonstrate the ability to repay student loans include (but
are not limited to):
• bankruptcy documents (particularly the Liabilities Page, listing all your creditors and the Order of Discharge, if
applicable) • a detailed itemized budget or financial plan • a recent demonstration of financial
stability/responsibility (e.g. a recent history of timely bill payment) • employment prospects • research that
suggests that you would be able to secure employment upon graduation • realistic financial projections/budget
based on the aforementioned research • employment trends • documentation of concerted efforts to improve
credit rating • proof of credit counselling, etc.
Role of Your Financial Aid Office:
Your financial aid office is available to assist you in answering any questions that you may have in regards
to preparing your submission. Students attending a school in Ontario must submit their requests for review
to the financial aid office at their school.
Where to Send Your Completed Form and Supporting Documentation:
• If you are attending a school located in Ontario, you must submit your completed credit check review
request form and a photocopy of your supporting documentation to the financial aid office at your
school.
• If you are attending a school located outside Ontario, submit your completed credit check review
request form and a photocopy of your supporting documentation by mail or by fax (please choose only
one method) to: OSAP Financial Eligibility Advisory Committee, 77 Wellesley St. W, Box 276,
Toronto, ON M7A 1N3 or (416) 325-3096.
Learning the Results of Your Credit Check Review
The review process takes approximately 8 weeks from the time your request is received. A decision letter
will be mailed to the address provided on your OSAP application.

In order for your credit check review to succeed, you must satisfy the OSAP Financial Eligibility Advisory
Committee of both of the following:
a.
The circumstances that led to your financial difficulties were beyond your control; and
b.
You are now financially stable, and will be able to repay your student loans.
Additional Instructions
In explaining how your current financial difficulties arose from circumstances that were beyond
your control, your response should include the following:
• a timeline of events that lead to your current financial difficulties; and
• documentation that proves your claims.
In demonstrating that you are financially stable, and will be able to repay your student loans,
your response should include the following:
• a proposed budget that outlines how you intend to repay any loans issued to you in addition to
your current debt load; and
• documentation that supports the plans you have outlined in your repayment plan.

The ministry uses relevant personal and tax information, including your Social Insurance Number, to administer the Ontario Student Assistance Program
(OSAP), including Queen Elizabeth II Aiming for the Top Scholarships. This includes determining eligibility; verifying the application, any loans approved, grants,
bursaries, or scholarships issued, and loans forgiven; maintaining and auditing your file; and collecting loans, overpayments, and repayments. The ministry
collects and uses this information under the authority of the Ministry of Training, Colleges and Universities Act, R.S.O. 1990, c. M.19, as amended, and R.R.O.
1990, Reg. 773, Reg. 774, and Reg. 775, as amended; and O. Reg. 268/01; the Financial Administration Act, R.S.O. 1990, c.F. 12, as amended; the Canada
Student Financial Assistance Act, S.C. 1994, c. 28, as amended; and the Canada Student Financial Assistance Regulations, SOR 95-329, as amended.
