Floor Load Placard Data Sheet
ALLOWABLE LIVE LOADS ON FLOORS
FOR BUILDING AT

PROCEDURE FOR ISSUANCE OF FLOOR LOAD CARDS
A)
INITIAL SEARCH:
The applicant must first go to the Records Bureau (Room 1100) and have a search made for microfilm records of Floor Load Cards issued in the past.
B)
IF RECORDS EXIST APPLICANT SHOULD:
1. Ask for photostatic copies of such records.
3. Give the certification letter to the structural examiner, along with a new application copied from existing records and certified by a licensed Illinois Architect or Structural Engineer. On this basis cards will be issued.
2. Engage an Architect or Structural Engineer to examine the existing floor framing conditions and issue a letter certifying that the loads per square foot shown on existing records may be applied safely on the existing floors.
4. After examiner has stamped and signed the cards and application, the applicant pays the fee (per Section 13-32-320 of the Chicago Building Code) and submits the application and duplicate card(s) to the final desk. The applicant keeps the photostatic copies of the records and duplicate cards.
The examiner keeps the application and certificate letter and submits them to the Records Bureau for microfilming.
C)
IF RECORDS DO NOT EXIST:
1. The examiner will give the applicant an application form.
3. The fee is established per Section 13-32-320 of the Chicago Building Code.
4. Cards and application will be stamped and signed by the structural examiner and the Chief Plan Examiner.
5. Application and calculations are kept by the examiner and submitted to the Records Bureau for microfilming.
6. If more than one card is required for the same floor, the Architect or Engineer should confer with the examiner by providing a key floor plan, showing the portions of floor areas affected.
2. The applicant should engage an Architect or Structural Engineer to investigate the floor capacities. All documents should be sealed and signed by a licensed Illinois Architect or Structural Engineer. Capacity compliance could be demonstrated in one of the following ways:

by submitting existing structural plans from existing microfilm records and providing one set of structural calculations;
by investigating existing structural framing and materials (see C.B.C. 13-120-100), and providing one set of structural calculations;
by testing existing structural assemblies per C.B.C. 13-120-110.
