1. Guidelines in filling forms and reporting
The role of Principal Investigator is to lead the research team and report the progress of research.
To enable the Project Investigator/researcher/ research coordinator/ reviewer to report, general
guidelines are given in filling the SAGA forms. These guidelines form the basis of this section.
1.1. SAGA Progress Report (Blue Form)
(a) Purpose
The purpose of the SAGA Progress Report is to allow the SAGA Secretariat to monitor the
progress of research by each of SAGA-funded project.
(b) Information required
In the SAGA Progress Report form, the Project Investigator is required to indicate the
milestones, timelines, and the actual expenditures incurred during the period.
The Principal Investigator (PI) is required to provide justification in the case of changes and
planned corrective actions for any variation of expenditure from budget. The PI is required to
indicate whether the project objectives have been achieved.
1.1.1 Guidelines in filling Project Description (Part A)
Please provide the complete CDC, Field and Sub-field code numbers as provided by Table (1),
(2) and (3) (see Appendix I).
As indicated, fill up the project title, Principal Investigator, Co-researchers, report date and date
of commencement of research project.

1.1.2 Guidelines in filling Milestone Achievement (Part B)
(a) Information required
Reporting should clearly relate to actual achievements to these milestones and outcomes. The
principal outcomes are those as in the SAGA objectives. Activities of research are monitored
according to the timelines and milestones achievement.
The Principal Investigator is required to indicate whether the milestones proposed in the original
project plan are achieved. The milestones (from 2004-2006) may provide an example as

1.1.3 Guidelines in filling financial report (Part C)
Vote 11000 – Salary and Wage
Under SAGA, almost all types of research related staff may be employed.
Vote 21000 – Travelling Expenses and Subsistence
To record claims by researchers while on official visit, including payment for personnel transportation, personal
subsistence, hotel and lodgings, toll and parking charges. For project staff appointed monthly, claims are according to
their eligibility following the Circular 2 Year 1992, with the exception of own transport at the rate of 60 cents/km. For
other researchers, travel expenses and subsistence are according to current government scheme.
Vote 22000 – Transportation of Goods
To record the expenses for the transportation of goods within and outside the country. This also include the cost of
operation and safekeeping during transportation of project equipment. Charges include courier, insurance for parcels,
storage charges and others.
Vote 23000 – Communication and Utilities
This vote records expenses relating to special services, examples post, telephone, telex, facsimile, cables and other. It
also includes charges for electricity, water, gas and other utilities. This vote includes installation and commissioning of
equipments and phone rentals
Note: Please specify type of services requested, frequency of requirement and expected cost incurred.
Vote 24000 - Rentals
To record all expenses for rentals, and leasing of land, building, vehicles, facilities and equipment.
Note: Must submit in the budget request, the type of rental needed, number and expected cost incurred. Rentals of gas
cylinders for liquid nitrogen, oxygen and others are included.
Vote 26000 – Supply of Raw Materials, and Materials for Repair and Maintenance.
To record expenses for all purchase of raw materials, and materials for repair and maintenance of assets, for example
spare parts. Payment for any services rendered for repair and maintenance should be placed under Vote 28000
Vote 27000 – Research materials & supplies (including animals, disposables, etc.)
To document purchases of all supplies and materials which are not categorized as assets. It does not include materials
and supplies under vote 26000. It encompasses items such as chemical reagents, consumables, glassware, animals for
research and other relevant items.
Note: Please provide details of cost and quantity of items required.
Vote 28000 – Maintenance and Minor Repair Services
To document expenses for external services to ensure all facilities and assets are in good working condition. This
includes cleaning and painting services.
Vote 29000 – Professional Services, Other Services & Hospitality
To document all services and hospitality rendered including professional services, photocopying,
printing, wages, allowance or other payments to those not appointed as project staff, hospitality and
honorarium to invited collaborators. Honorarium is restricted to respondents, e.g., payment for
blood samples in a medical project.
Note:
Please provide details of services or hospitality rendered, inclusive of the rates and mode of calculations.
Vote 35000 – Other Fixed Assets
To document all expenses for all items categorized as fixed assets.
Note: Please itemize all equipments to be bought under this vote and provide justifications, specifications, and estimated
costs.

Part D-Progress Report
Please indicate your significant findings
Part E-Research status
Please tick appropriate box to indicate your research status. According to 4.8 of SAGA
Fund Agreement the researcher should have at least 3 publications relevant to the research
in internationally ranked journals.
(b) Responsibility
The Principal Investigator of SAGA-funded project should complete the SAGA Progress
Report.
(c) Frequency of reporting
There will be two SAGA Progress Report of the same form to be submitted at the end of 8th
months and 20th months of the project during the first term (2 years).
(c) Expectation
It is hoped to achieve the following:(1) There is an advancement in scientific information
(2) That this fundamental science is new, good quality of work and value towards
contribution to science
(3) Financial status
(4) Research is on schedule
(5) That there is capacity building and expertise building up
(e) Submission procedure
One copy of SAGA Progress Report is to be mailed to

1.2 SAGA Milestone Achievement Modification Report (Yellow Form)
The purpose of the Special SAGA Milestone Achievement Form is to provide a detailed
information only in the case the proposed Milestone could be achieved.
In the event that the milestones are not achieved by the proposed deadline, the Principal
Investigator is required to:

provide reasons for the non-achievement;

suggest adjustments to the project approach; and

revise the project schedule and budget.

1.3 SAGA Progress Project Monitoring (Green Form)
The purpose of the SAGA Progress Project Monitoring Report is to allow the SAGA Secretariat
to assess the status of SAGA-funded projects, specifically, the achievement or otherwise of the
original project objectives
1.3.1 Guidelines in filling SAGA Progress Project Monitoring Report
Filling the Part A is the same process as in previous SAGA Progress Report form. In Part B, the
name, contact number of reviewer and the date of review are required.
The evaluation is carried out by filling Part C has been structured according to the number 1,2,3,4, 5,
6, 7, 8 and 9. The evaluation indicators are given in a structured form. The weighted scores are
given in terms of numbers from 1 meaning inadequate to 5 meaning very good marks.
For
suggestion to be taken, filling Part D by ticking according the appropriate box . In ticking the box
Project modification, some information is required in Part E. In ticking Project termination, reasons
are required in Part F. In ticking box Project to be reviewed again once additional information has
been obtained from Principal Investigator, please provide the specific information by filling Part G.
In the case of space for writing is insufficient, please provide your writing on another your own
sheet of paper.
In ticking box Payment to be deferred until next assessment, reviewer can give the comments by
filling Part H. Part I is blank for SAGA official action.
(a) Role and responsibility
The role and responsibility research centre coordinator are to assess and fill the SAGA Progress
Monitoring Form. Assessment can be done by referring to SAGA Progress Report which has
been completed by Researcher/Principal Investigator or if necessary by on-site visits to the
recipient research institution/academic institution. The completed form must be submitted to
SAGA secretariat.
(b) Frequency of reporting
The SAGA Progress Project Monitoring Report is to be submitted within the year of the
proposed research, as outlined in the original plan. It is carried out twice per research term.

c) Expectation
It is hoped to achieve the following:(1) It has achieved all the proposed objectives, milestones and work schedule
(2) Progress in research
(3) Proper utilization of resources, time and money

1. 4 Guidelines in filling SAGA Fundamental Research Reviewer (Red Form)
(a) Purpose
The purpose of SAGA Fundamental Research Reviewer is to give a final assessment of the research
by SAGA recipients.
(b) Role and responsibility
The role of reviewers is to critically assess the performance of research by principal investigator
report in line with the research objectives and overall objectives of SAGA. The reviewer is an expert
in the field.
SAGA Fundamental Research Reviewer form is a guide to assess the research progress and
performance.
Filling the forms is straight forward and self-explanatory for the part A and B. Filling part C
requires a careful assessment, similarly for part D and E. F and G require some comments by
reviewer. If space for writing is insufficient, please provide your writing on another your own sheet
of paper.
The SAGA Progress Project Monitoring Report is to be submitted within the year of the proposed
research, as outlined in the original plan. It is carried out twice per research term.
(c) Expectation
It is hoped to achieve the following:(1) Publications in refereed journals in terms quality and quantity are met
(2) The project objectives, milestones and financial status are achieved
(3) By doing this fundamental research there is an advancement in science knowledge
(4) That this fundamental science is one of quality work and value towards contribution to
science
(5) That there is capacity building and expertise building up
(d) Submission procedure
The completed SAGA Fundamental Research Reviewer’s form is to be given to:

1.5 Guidelines in filling SAGA End of Project Report (Pink Form)
(a) Purpose
The purpose of SAGA End of Project Report is to give the end results of the project by SAGA
recipients.
(b) Role and responsibility
The role of researcher/PI is to write end results of project report.
The SAGA End of Project Report form is a guide to assess the research performance.
It is designed to extract the information on projects that is ending.
The SAGA End of Project Report is to be submitted within three months of the completion of the
research project. It is carried out once per research term.
c) Expected Outcomes
It is hoped to achieve the following:(1) contributions in the project
(2) achievements and new knowledge
(3) project work completion on schedule
(4) proper utilization of resources, time and money
(5) research on progress
The project is really successfully finished, has met its objectives.
(d ) Frequency of reporting
The report is required only once.
(e) Submission procedure
The completed SAGA End of Project Report’s form is to be given to:

1.6 Benefits of Project Report (White Form)
(a) Purpose
The purpose of SAGA Benefits of Project Report is to give financial and non-financial benefits of
the research as carried out by SAGA recipients. The financial benefits are profits and growths while
the non-financial benefits are motivation, total service, flexible and satisfaction.
(b) Role and responsibility
The role of researcher/PI is to write benefits in the projects.
The SAGA Benefits of Project Report is a report to assess benefits as the result of the research done
by working cost-benefits analysis. “What has been produced” and “what has been used up” should
be mentioned.
The SAGA Benefits of Project Report is to be submitted within three months of notification by
SAGA Secretariat. It is carried out twice per research term.
(c ) Expectation
The benefits of the project are mainly to its own institution, society, state, government of Malaysia
and the contribution of science to the world. The deliverables from this project are expected to
mention.
(d )Frequency of reporting
The report is required only once.
(e) Submission procedure
The completed SAGA Benefits of Project Report’s form is to be given to:

1.7 Benefits Assessment Report (White Form)
(a) Purpose
The purpose of SAGA Benefits Assessment Report is to give a final assessment of the research by
SAGA recipients.
(b) Role and responsibility
The role of reviewers is to recommend the necessary actions. The reviewer is an expert in the field.
The SAGA Benefits Assessment Report is a report to give the benefits assessment.
The benefits assessment to give an overview of all the benefits as described by the researcher,
Principal Investigator and other peer groups.
The SAGA Benefits Assessment Report is to be submitted within three months of receiving the
Benefits of Project Report. It is carried out twice per research term.
(c) Expected Outcome
Satisfaction of the project and the whole exercise is beneficial.
(d )Frequency of reporting
The report is required only once.
(d) Submission procedure
The completed SAGA Benefit Assessment Report is to be given to:
