2 Step Process
1st Step:
Is this dose a SAFE/RECOMMENDED dose for THIS child?
Look up what IS SAFE/RECOMMENDED dose…
For example:
Amoxicillin (for otitis media) 40 mg/kg/day
Next – how much dose the child weigh?
Child weighs 31 lbs
Convert lbs to kg
31 divided by 2.2 =14.0 kg
40 mg/kg/day
40mg x 14.0 = 560 mg per day.
This child is prescribed
280 mg PO BID
Is this appropriate?
YES --- 280 + 280 = 560mg per day
2nd Step-
How much are you going to give?
What CONCENTRATION DO YOU HAVE??
or
250 mg/5ml
Determine amount per ml
250 divided by 5= 50 mg per ml
Determine amount to administer
280 divided by 50 = 5.6 ml
Give 5.6 ml
400mg/5ml
400 divided by 5 = 80 mg per ml
280 divided by 80= 3.5 ml
Give 3.5 ml
REMEMBER it’s VITAL you look at concentration EACH time!!!
