Registration Application Form Instructions
This document covers the instructions for completing the “Registration Application Form” for
the Manitoba Beekeepers’ Association.
REGISTRATION APPLICATION FORM
Item 1: Print the name of the Honey Producer or Beekeeper desiring to be registered. For
Business, Print the name usually used for promoting or operating the Honeybee business.
Item 2: Identify the number of Honey producing colonies kept the previous calendar year.
Include all planned splits, additions, or new purchases into the current year count.
Item 3: Use the 3 or 4 digit beekeeper registration number provided by the province at the time
you registered with them.
Item 4: Complete the one and only one portion that best describes your operation.
a) If you alone own, operate, and benefit from the business under your personal, or business
name, then complete portion “a”. Print your name in this area.
b) For those informally operating the business as a family unit, where a spouse is sharing
ownership, operation and benefits, then complete portion “b”. You may or may not have
registered a business name, or other official documents covering the partnership
arrangements. Print the name of your spouse.
c) For those operating the business with their spouse, siblings, in-laws, parents, children, or
others, who purchase, own, operate and share revenues, then complete portion “c”. You
should have some formal documents covering the sharing arrangements, including
registration of the business name. A copy of the registration document is to be attached to
this application form. Please print the names of all partners, their share of investment, and
percent profit share.
d) For larger operations more formally structured, where several others are involved in
ownership, operation and sharing revenues, portion “d” applies. Identify the date the
business was incorporated, whether operated under Provincial or Federal charter, etc.
Print the names of all officers and directors, each person’s address, and identify whether
each has voting or non-voting status with the business. Note, a copy of your Articles of
Incorporation, and all other articles of the applicant, (i.e. Annual Return of Information,
etc.), and including a recently issued Certificate of Status from the Companies Office,
1010-405 Broadway, Winnipeg, MB, R3C 3L6, must be sent along with the registration
form.
Item 5: Identify whether or not the applicant has any type of connection to the beekeeping
operation(s) of any other registered producer(s) in Manitoba. If the response is “yes”, please print
the name(s) and the type of connection.
Please Note: For completing items 5 and 6, anyone having any connection, financial or
otherwise, direct or indirect, with any other registered producer is ineligible for membership,
according to Section 22 of the MBA bylaws, which states:

22.Where two or more persons operate colonies of bees, in partnership, or in circumstances in which there is a
sharing by them, whether familial, communal, or otherwise, of facilities, labour, or services provided, directly or
indirectly, by all or any of them, or pursuant to a contract jointly or severally with the same corporation, firm or
individual for the purpose of this exemption any colonies of bees operated in any year by one of these persons shall
be deemed to have been operated in that year by the other or others.
Item 6: This section deals with the identification of whether or not any other registered
producer(s) in Manitoba has any type of connection to the applicants beekeeping operation. If the
response is “yes”, please print the name(s) and the type of connection.
Item 7: This area confirms the documents to be included along with the application. A
Designated Representative form must be included if you have checked the Partnership,
Corporation or Hutterite Colony categories. The sole proprietor is automatically the designated
representative of his/her operation.
Item 8: Leave this for MBA office to record the date the application is received.
Item 9: Leave this for MBA office to verify receiving all related documents.
AFFIRMATION OF APPLICANT
This page of the Registration document needs to be signed by the applicant(s) in the presence of
a Commissioner of Oaths, Notary Public, or other Provincially Approved person who will affirm
the correctness of the details within the application. The page is separated into two areas.
(Individual or Partnership). The top portion of the page is to be completed by an individual (sole
proprietor) or two members of a Partnership. In the space provided, print the applicants’ name,
then, print one descriptive item (i.e. any one of city, town, hamlet, municipality, etc), then the
appropriate name of the city, town, hamlet, municipality, etc., in the space provided.
(President or Director of Corporation). The lower portion of the page is designed for approved
signators of Corporations or Hutterite Colonies. As above, the applicant is to print their name,
followed by printing one descriptive item (i.e. any one of city, town, hamlet, municipality, etc),
then the appropriate name of the city, town, hamlet, municipality, etc., in the space provided.
