Provide a description on how you collect income data and how it is compared to federal poverty guidelines. Examples: An organization that primarily provide affordable housing services for the low/moderate collects incomes on each individual resident, and it is substantiated by; most recent W-2 or current paystub amortized over a full year. Organization should provide specific information on % of their population served that meets the low or moderate income demographics, as published by HUD standards in the geographic area served. If providing services to low or moderate income students include the school(s) data on # or % of students who receive free/reduced lunch benefits, if providing services to these children specifically, demonstrate how your organization identifies the recipients. 4) Provide written Executive Summary of financial condition (500 words or less) a.
