A study of extreme
prematurity in 2006
EPICure 2 LABOUR WARD LOG
To be completed for all births 22+0 to 26+6 weeks and all registered live births <22 weeks
Stillbirths and neonatal deaths beyond 27+0 weeks of gestation should be reported to CEMACH using the standard CEMACH PDN Form
GUIDANCE FOR COMPLETING FORMS
1. Each birth should be entered on a new row, i.e. 2 rows for twins.
2. The combined EPICure/CEMACH for PN:E2 should be completed for all births entered
on this log.
3. Fetal or neonatal deaths of these births do not need the standard CEMACH PDN form
completed.
4. Umbilical cord collection: enter on form:

placenta and membranes sent to pathology
piece of umbilical cord only sent to pathology
piece of umbilical cord retained on Labour Ward in formalin
placenta and membranes discarded, no cord sample taken
Please label any sample of placenta or cord sent to the laboratory with a bar coded study label
in addition to your routine hospital label. Please also send a labelled study request form (kept in
the yellow study folder) in addition to your routine pathology request form.
At the end of each month please send forms, whether or not any births have been
entered, to the EPICure Study Centre at Homerton University Hospital, using a
Freepost label from the back of this folder or fax to 020 8510 7850.
Please retain a copy in your study file.
