Occuloplastics Surgical Fellowship

The Post
This is a clinical fellowship at Birmingham and Midland Eye Centre, with the
Oculoplastics Consultants, Mr Aidan Murray and Mr Omar M Durrani. It is suitable for
Post CCST UK Trainees or suitable qualified overseas trainees offering opportunities to
gain experience in Oculoplastic and Lacrimal and Orbit Surgery.
Trust Profile
The Trust is one of the largest teaching Trusts in the United Kingdom with a reputation
for excellent, friendly staff who provide high quality care from three hospitals:



City Hospital in Birmingham
Sandwell General Hospital in West Bromwich
Rowley Regis Community Hospital in Rowley Regis
The Trust was established in April 2002 and has an income of £365 million. It employs
almost 6400 WTE staff, has circa 1000 beds and serves a population of over 500,000.
Sandwell General Hospital and City Hospital are busy acute hospitals providing many
specialist services and a broad range of emergency services, including Accident &
Emergency at both sites. Rowley Regis Community Hospital provides rehabilitation and
a wide range of outpatient and diagnostic services.
The Trust is a key partner along with local PCTs and local authorities in the “Right Care
Right Here” programme which seeks to deliver an ambitious redevelopment of local
health services. This has the backing of the West Midlands Strategic Health Authority
and has been approved as a national priority scheme by the Department of Health.
Following a very successful public consultation, implementation of the programme is
underway with a wide range of secondary care services now being provided via new
models of care in community locations. The programme includes one of the largest
investments in the UK in new facilities in both the acute and community sectors.
Included within this is a brand new state of the art acute hospital which has both
business case approval and planning permission. The new hospital is set to open in
2015/16. This will create some of the largest clinical teams in the country on a single
site.
The Trust’s current strategy focuses on the period leading up to the new hospital with
an emphasis on driving clinical integration by reconfiguration of services between the
existing sites, strengthening key specialties and on quality and productivity
improvement. In the light of its strategic, operational and financial strength the Trust is
applying to become a NHS Foundation Trust.
The Trust is a pioneer in developing new and more effective approaches to staff
engagement through its “Listening into Action” programme which harnesses the energy
and ideas of front line staff to improve services. This is the largest programme of its
kind in the NHS and has received widespread national recognition.
The Birmingham Treatment Centre opened on the City Hospital site in November 2005.
This £35m development provides state of the art facilities for one-stop diagnosis and
treatment. It includes an Ambulatory Surgical Unit with six theatres, extensive imaging
facilities, an integrated breast care centre and teaching accommodation.
A new £18m Emergency Services Centre opened on the Sandwell site in April 2005. It
incorporates a comprehensive A&E facility, Emergency Assessment Unit and Cardiac
Care Unit. The model of care has been developed with primary care to provide a fully
integrated service.
The Trust hosts the Birmingham and Midland Eye Centre which is a supra-regional
specialist facility, as well as the Pan-Birmingham Gynaecological Oncology Centre,
Birmingham Skin Centre and Sickle Cell and Thalassaemia Centre.
Aside from being one of the largest providers of acute patient services in the Midlands,
the Trust also has a substantial teaching and research agenda with several academic
departments including rheumatology, ophthalmology, cardiology, gynaecological
oncology and neurology.
BIRMINGHAM AND MIDLAND EYE CENTRE (BMEC)
The Eye Centre has been purpose built for ophthalmology and opened in March 1996
and is based at the City Hospital site. The Eye Centre includes the following; Academic
Unit, Accident and Emergency Department, Out-patient Suites, 4 operating theatres
(including one on the Day Surgery Unit), Medical Photography (fluorescein and
indocyanine green angiography, anterior segment digital photography) Visual Function
Department (visual fields, electrodiagnostics, ultrasound, HRT and OCT), Orthoptic
Department, Refraction Service (including contact lenses, low vision aids, and
dispensing services), Artificial Eye Service, Ophthalmic Ward with 20 in-patient beds,
Day Surgery Unit with 21 adult places and 5 paediatric places. There are 28 Consultant
Ophthalmologists and 27 have a commitment at the Eye Centre. The current Clinical
Director is Mr Shashi Aggarwal. Subspecialty interests represented include cornea and
external eye disease; vitreo-retinal surgery; medical ophthalmology including diabetic
eye disease, macular degeneration, retinal vein occlusion; glaucoma; uveitis; neuroophthalmology; oculoplastic and orbital surgery; ocular motility; paediatric
ophthalmology; retinoblastoma; genetic eye disease.

JOB SUMMARY:
This post is designed to provide sub speciality training on Oculoplastics, Lacrimal an
Orbital Surgery. This post is recognised for sub-speciality training in Oculoplastic and
lacrimal surgery but it is not recognised towards CCST, nor is it recognised by the
Regional Postgraduate Dean for training purposes.
The effect of these strictures is that the post is not suitable for a UK Specialist Registrar
who is hoping to use this fellowship towards CCST. It would only be suitable for such a
person after the award of CCST. Overseas candidates, if appointed, will have to obtain
a work permit (this usually takes one to two months).

Clinical Responsibilities
The post comprises of a full time training in Oculoplastics, Lacrimal & Orbit surgery
under the supervision of Mr A Murray & Mr O M Durrani, Consultant Ophthalmic
Surgeons. The post holder is expected to work closely and flexibly with the oculoplastic
ASTO (internal fellow) and there will be a rotation of the 2 fellows after 6 months. The
successful candidate will also be expected to do one casualty session per week.
The Birmingham and Midland Eye Centre forms the hub of a system of hospitals within
Birmingham. The spoke units are at Good Hope Hospital, the Birmingham Heartlands
Hospital and Selly Oak Hospital. Referrals to the Birmingham and Midland Eye Centre
Oculoplastic, lacrimal, and orbital Unit occur from the spoke hospitals, from patients
attending the Casualty unit at the Birmingham and Midland Eye Centre and also from a
substantial region of the Midlands extending in the north to Stoke on Trent, the south to
Hereford in Worcestershire, in the West to Aberystwyth in Wales and many points in
between. The total catchment area is probably in the region of 3-4 million persons.
The post is designed to provide a comprehensive training in Oculoplastic and Lacrimal
surgery and comprises specialist oculoplastic theatre sessions and clinics under the
supervision of the Consultants. As experience is gained, increasing responsibility is
given. The fellow will have considerable exposure to Orbital surgery and the
management of Orbital conditions; however it will not provide a complete training in
orbital surgery. The successful candidate will expected to act as a liaison between junior
doctors, other Consultant Ophthalmologists, Consultant Plastic/Maxillofacial/ENT
Surgeons, Consultant Dermatologists and Pathologists and the Oculoplastic
Consultants. Up-keep of the Oculoplastic database is a key role of this post and will
inevitably create research potential. The post holder will be expected to contribute to
teaching, clinical audit and research. Clinical audit is established on a Birmingham-wide
basis with six meetings a year at the Birmingham and Midland Eye Centre. The
supervising Consultants will arrange a formal assessment of progress during this
attachment.

GENERAL INFORMATION
Library & Information Services
The two multi-disciplinary libraries at City and Sandwell Hospitals include a large IT
section, with access to all standard databases, together with Internet access. There are
also slide and video programmes and interactive CD ROM facilities. 24-hour access is
available to all medical staff.
Research
The post holder will be encouraged to undertake research. Clinical and laboratory
facilities and support, including statistical advice, are provided within the Trust and at
the University of Birmingham sub-units on the City Hospital Site.
All research undertaken should conform to the rules of Good Research Governance
and all research projects involving patients or their records (including those originating
elsewhere) must have approval from the Research Ethics Committee and Research
and Development Directorate.
Teaching
The post holder will be required to participate fully in the education and training of
medical students, trainee doctors, paramedical, nursing and other appropriate
personnel.
There are modern Education Centres on the City and Sandwell Hospital sites, which are
the focal point for teaching and training. Postgraduate clinical meetings are held in the
Centre on a daily basis.
GENERAL CONDITIONS OF SERVICE
1. The post is a Trust appointment based on the General terms and conditions of
the Whitley Council.
2. The appointment is for 6 months with a further extension of 6 months subject to
performance.
3. The post has a salary of £32,000.
4. Annual leave entitlement for the post is five weeks per annum plus Bank
Holidays. Annual leave is granted with the approval of the Supervising
Consultant and Divisional Director.
5. The appointment is subject to a satisfactory Occupational Health clearance and
Criminal Records Bureau check.
6. Any candidate who is unable for personal reasons to work whole time will be
eligible to be considered for the post.
7. It is the responsibility of all medical staff to ensure that they are appropriately
registered with the General Medical Council. If registration lapses employment
may be terminated.
8. All employees are expected to comply with appropriate Trust Health and Safety
policies.
9. Under the Statutory Instrument 778 Health and Safety Radiation Protection of
Patients, employers have an obligation to ensure that any person clinically or
physically directing medical radiation exposures have received adequate training.
It is therefore, expected that you should submit to the Medical Staffing
Department a copy of your certificate or attend an appropriate course.
