EQUIPMENT LIST FOR
EQUINE CREDENTIALS
COMMITTEE REVIEW
Equine Dental Specialty
Information for Trainees
1. Approval by the Credentials Committee of a list and images of the equipment,
instruments and supplies available at the trainee’s facility is required for Part-Time
Residents. Residents at AVDC-approved residency sites are not required to provide this
list, because it is included in the Residency Site approval process.
2. Use of the Pre-Approval process is recommended, so that any deficiencies can be corrected
prior to submitting a Credentials Application.
3. Complete the table on page 2. Note that it is not necessary to have every item listed in the table
– the purpose of Credentials Committee review is to confirm that a comprehensive set of
instrumentation is available to the applicant, as would be expected of an entry-level veterinary
dental specialist. If you do not have an item, write None in the How Many? column, and
include a comment in the Comments column if you have equipment or instruments that serve
that purpose.
You can submit a maximum of 20 images, maximum of 2 images per category, using the
following categories:
A. Operatory or ambulatory vehicle, table, chair, lights, stocks, and treatment area.
B. Sedation and monitoring equipment.
C. Major dental equipment and hand pieces (including high speed unit, scaler, electrosurgery unit, occlusal adjustment instruments, laser unit if available).
D. Radiology unit, sensors/films/chair-side developer, light box.
E. Periodontics, including hand instruments, supplies and home care products.
F. Endodontics, including instruments and supplies.
G. Restorative, including instruments and supplies.
H. Oral surgery, including instruments and supplies.
I.
Orthodontics/occlusal adjustment, including instruments and supplies.
J.
Miscellaneous and anything not able to be included in the previous categories.
4. Electronic submission as an Equipment List document via DMS is required. Read the DMS
User’s Guide for more information. Name this completed file: YourLASTNAME,FirstName
CredEquip. Name the images: YourLASTNAME,FirstName CredEquip image A1 etc. as in
item 3, above. Images can be submitted as .jpg or Acrobat.pdf files.
The Credentials Committee Review Team may recommend approval, request clarifications
(describing the specific clarifications requested), or recommend non-approval of the list
(describing specific deficiencies). If an Equipment List is not approved, the trainee may submit

an addendum consisting of a list (and images if appropriate) of equipment or materials acquired
to correct the identified deficiencies.
