Clinical Performance Goals
Strategy
Observe the ways that nursing
staff provide comfort care through
positioning, physical care,
medication management,
emotional support, and other
techniques.
Assume more nursing tasks each
week that deal with providing
palliative care.
Review the on-site resources for
staff and families regarding
methods of comfort care; and also
search the internet and additional
literature to learn more on the
subject.
Solicit ideas for time management
from my preceptor.
Utilize an organized worksheet for
each patient to manage the nursing
care tasks and patient specifics
throughout the day.
Discuss my daily plan with my
preceptor before beginning each
shift.
Review the efficiency and quality
of my nursing care at the end of
each day with my preceptor.
Observe Hospice staff interactions
with patients and their families.
Review literature on this topic
recommended by my preceptor
and search the internet for further
reading.
Interact regularly with patients and
their families to improve my
comfort level in this setting.
Educational Goal
Strategy
Demonstration of Goal Attainment
 I will discuss new learning and
experiences with my preceptor.
 My preceptor will state a level of
confidence in my basic knowledge
of palliative care measures used in
Hospice setting.
 I will confidently be able to name
and recite general information about
at least 10 medications used for
palliative care.
 I will be able to name 10 additional
techniques used for palliative care
and the reasoning behind using these
techniques with Hospice patients.




Have the ability to safely and
confidently manage a full
assignment of patients by the end of
the quarter.
Complete tasks on time and in a
non-rushed manner.
Noting through journaling a marked
improvement in my confidence in
offering psychological support.
My preceptor will be able to remark
on my improvements and skills in
this area.
As I will be witnessing the End of Life care of many
cancer patients in the Hospice setting, I would like to
learn more about what these individuals have likely gone
through over the months or years leading up to Hospice
Demonstration of Goal
Attainment
 Discuss
information
learned with my
preceptor and my
diagnosed with
lung cancer.
care in their efforts to treat the cancer. I am very
interested in what these families and patients have gone
through with their disease.
This will include researching literature about
chemotherapy, radiation therapy, naturopathic remedies,
physical therapies, and many more to be discovered,
including new trials of therapies. As cancer is such a
varied diagnosis, I will focus my research on the
treatment trajectory for lung cancer patients. In order to
best understand the course of therapy, I will also delve
more into the pathophysiology of this specific cancer and
compile general information and statistics about the
disease.

Student Expectations of Preceptor
That we will have a brief check-in at the beginning and end of each clinical day to review
my goals for the day and strategies to attain them, in addition to reviewing my preceptors
vision for the clinical experience on the particular day.
Be open and honest with me and provide ongoing feedback.
Hold me accountable for my actions.
Challenge me with questions regularly.
Role-model the proper ways to perform various nursing skills.
Encourage me to try new things. I am a visual learner. I learn best by first watching then
by getting hands-on experience.
Find a variety of learning opportunities for me and take advantage of teachable moments
(including seminars, classes, staff meetings, and trainings available)
When she is comfortable, allow me to function as independently as possible.
Be professional and engage in her role as a preceptor.

Preceptor Expectations of Student
Act and dress professionally, and be on time.
Respect the patient and family, the staff that I am working with, and myself.
Communicate my comfort zone on patient care and help her to provide the education
skills that I need to be a skilled professional nurse and caregiver.
Each shift she expects me to gain skills. As my preceptor becomes comfortable with my
skills, she will allow me to perform with more independence.
Once I am proficient with one patient, she will increase by one additional patient, so that
at the end of our time I should be able to plan and care for 5 or 6 patients in a time
efficient manner.
She recommends that I read “The Final Gift”.
I will attend the Hospice Pain Management course with her offered by Evergreen
Hospital (6 hours CEU’s).
When I do not understand something or want further information, I will not hesitate to
ask questions. She expects me to always ask before doing.
She hopes to help me build my confidence in my skills, so that I will be very successful
in my nursing career.
