We are confident that our website will provide clear and concise help and give our patients the
information they require in an easy and convenient format. It has been designed with the patient’s
needs at the forefront of everything, from checking surgery times to letting us know what you think

Serving You
Our dedicated team are here to treat those minor ailments that occur as well as providing specialist
management of long-term conditions and clinics covering a wide range of healthcare issues. The
technology also means you can now do a lot of things from the comfort of your home such as order
a repeat prescription or cancel an appointment.

Once registered, patients and healthcare professionals work together to ensure the most appropriate
care is provided. This partnership philosophy extends even further and our active patient group
exists to make sure that patient needs and the practice offering are always heading in the same
direction.
Access & Parking
The surgery is accessible to wheelchair users and has disabled parking bays close to the building
with a ramp to the front door. Patients unable to open the front door should use the bell to request
assistance from a member of staff. The patient toilet is fully equiped for the needs of disabled users.
There is a convenient public car park just a short distance from the surgery in Lower Lane. Patients
are asked not to park on the surgery forecourt as this may cause an obstruction to vehicles required
for a medical emergency.
Smoking & Mobile Phones
Smoking is not permitted anywhere on the surgery site and we kindly request that you refrain from
using your mobile phone whilst in the surgery building.

When We Are Closed
When the surgery is closed and you
require medical attention, you should contact the NHS 111 service by dialing 111 from any
phone. The out of hours service is provided by West Hampshire Primary Care Trust in partnership
with Hampshire Ambulance Service.

In a genuine emergency you should call 999. Chest pains and / or shortness of breath constitute an
emergency.
Appointments



Routine/Non Urgent Appointments - please avoid telephoning the surgery between 08:00
and 10:00 for routine appointments as this is when the telephone lines are generally at their
busiest with urgent enquires.
Urgent/Same Day Appointments - if you are not able to wait until the next routine
appointment, we have procedures to ensure that, where necessary, you will be seen by a
healthcare professional on the same day. In some cases, your initial treatment may be given
by a member of the nursing team; they may refer you to see a GP after appropriate
assessment.
Emergency Appointments - emergencies are seen as soon as possible. Please ring and
speak to a receptionist who will ensure that appropriate arrangements are made for you to be
seen. In life threatening situations please ring 999. The surgery may not have all the
appropriate resources to deal with such situations.
Intimate Examinations/Chaperones
If you require a chaperone to be present during intimate examinations, please ask in advance if you
require the practice to provide this, or ask a friend or relative to accompany you. Please bear in mind
that personal information will be discussed during your consultation.

Cancellations
If you cannot attend an appointment for any reason please inform us as soon as possible to allow us
to offer the appointment to someone else.
Home Visits
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and
facilities available, we do appreciate this is not always possible. In this respect, if you do need a
home visit, you can help us by calling reception before 10:00.
You should only request a home visit if you are housebound or are too ill to visit the practice. Your
GP will only visit you at home if they think that your medical condition requires it and will also
decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable
details to enable the doctor to schedule house calls
You can also be visited at home by a community nurse if you are referred by your GP. You should
also be visited at home by a health visitor if you have recently had a baby or if you are newly
registered with a GP and have a child under five years.
Sickness Certificates
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your
employer may however require you to complete a self-certification form (SC2) which is available
from your employer or on the HMRC website (www.hmrc.gov.uk/forms/sc2.pdf) .
Evidence that you are sick
If you are sick for more than seven days, your employer can ask you to give them some form of
medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate, now
called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are
sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist.
Your employer will decide whether or not this evidence is acceptable. If your employer has any
doubts, they may still ask for a medical certificate from your GP.
Statement of Fitness for Work - ’Fit Note'
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you
return to work sooner by providing more information about the effects of your illness or injury.

Repeat Prescriptions
Patients on long-term medication can use the repeat slip attached to their last prescription to re-order
or via email at [email protected]
Alternatively you can order online via the link at the top of this page. New users are required to
complete a simple registration.
Dispensing
We are able to dispense medication to all of our patients living more than 1.6km (a mile) from any
chemist. The dispensary is open from 08:00 - 18:30 providing a timely and professional dispensing
service. We also provide a delivery service for those patients who are housebound and unable to
collect their medication from the surgery. Mrs Hema Joshi, Dispensary Manager, and her staff are
available to offer advice about this and other services the dispensary can offer. We are able to
dispense all private prescriptions at the surgery, no matter where you live, please contact the
dispensary for more information
Medication Reviews
Patients on repeat medication will be asked to see a doctor, nurse practitioner or practice nurse at
least once a year to review these regular medications and notification should appear on your repeat
slip. Please ensure that you book an appropriate appointment to avoid unnecessary delays to further
prescriptions.
Please allow two full working days for prescriptions to be processed and remember to allow extra
time for weekends and bank holidays.
Prescriptions Charges and Exemptions
Extensive exemption and remission arrangements protect those likely to have difficulty in paying
charges (NHS prescription and dental charges, optical and hospital travel costs).
The NHS prescription charge is a flat-rate amount which successive Governments have thought it
reasonable to charge for those who can afford to pay for their medicines. Prescription prepayment
certificates (PPCs) offer real savings for people who need extensive medication.
NHS charges
These charges apply in England only. In Northern Ireland, Scotland and Wales prescriptions
are free of charge.




12-month prepayment certificate (PPC): £104.00
3-month PPC: £29.10
If you will have to pay for four or more prescription items in three months, or more than 15 items in
12 months, you may find it cheaper to buy a PPC.



Management of Long Term Medical Conditions - monitoring & review of established
health problems are carried out by a doctor or practice nurse. You may request to see a GP or
nurse at any time; however your annual review will be arranged by written invitation.



Cervical Smears - women between 25 and 64 are advised to have a regular cervical smear.
This is by appointment with a practice nurse. You will be advised when your smear is due by
letter and you will be informed of your result in writing.
Sexual Health - we offer a comprehensive sexual health screening service with our practice
nurse, Lynn Watkins. All advice and treatment is strictly confidential and is available to
patients of all ages. Routine screening for chlamydia is also offered.
Minor Surgery - a limited range of minor surgical procedures are available.

In addition to the main building, the annexe is used to house a variety of services provided by the
midwives, health visitors, community nurses, speech therapist, dietician and counsellors.
Vasectomy Services
Vasectomy Services are provided at the Surgery by Dr David Procter. If you are considering a
vasectomy please make an appointment with a GP to discuss your options. Should you decide that
surgery is the best option for you then an appointment will be made with Dr Procter.
Quit Smoking
We offer a comprehensive 'Quit' smoking service at the surgery. Our Practice Nurses will guide &
support you through those vital first few weeks of your journey. If you wish to find out more about
this service and how we can help you then please make an appointment with one of the nursing
team.

Childrens Immunisations
Immunisation is the best way to protect your child against infection and disease. It is vitally
important that you attend for all stages of the various vaccination schedules. If you miss an
appointment you can call the surgery and make an appointment with the practice nurse.
The vaccination programme is fully supported by the GP's and if you have any concerns about any
of the immunisations you can discuss them in more detail with a member of the nursing team.

Non-NHS Services
Some services provided are not covered under our contract with the NHS and therefore attract
charges. Examples include the following:






Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
Insurance claim forms
Passport signing
Prescriptions for taking medication abroad
Private sick notes
Vaccination certificates
The fees charged are based on the British Medical Association (BMA) recommended scales and our
reception staff will be happy to advise you about them along with appointment availability.

Test Results
When your Doctor recommends further investigations such as a blood test or xray you will be
advised how long you should expect to wait for the results and how they will be conveyed to you.
Our reception staff are not qualified to comment on the outcome of a test and are only able to report
on test results that have been viewed by the requesting Doctor. Our telephone lines are open
between 2pm & 4pm.
Please note that we do have a strict policy regarding confidentiality and data protection. In this
respect we will only give out results to the person they relate to unless that person has given prior
permission for their release or if they are not capable of understanding them.
Blood Tests
Blood Tests
A blood test is when a sample of blood is taken for testing in a laboratory. Blood tests have a wide
range of uses and are one of the most common types of medical test. For example, a blood test can
be used to:



assess your general state of health
confirm the presence of a bacterial or viral infection
see how well certain organs, such as the liver and kidneys, are functioning
A blood test usually involves the phlebotomist taking a blood sample from a blood vessel in your
arm. and the usual place for a sample is the inside of the elbow or wrist, where the veins are
relatively close to the surface. Blood samples from children are most commonly taken from the back
of the hand. The child's hand will be anaesthetised (numbed) with a special cream before the sample
is taken.

X-Ray
X-Ray
An X-ray is a widely used diagnostic test to examine the inside of the body. X-rays are a very
effective way of detecting problems with bones, such as fractures. They can also often identify
problems with soft tissue, such as pneumonia or breast cancer.
If you have a X-ray, you will be asked to lie on a table or stand against a surface so that the part of
your body being X-rayed is between the X-ray tube and the photographic plate.
An X-ray is usually carried out by a radiographer, a healthcare professional who specialises in using
imaging technology, such as X-rays and ultrasound scanners.

Registration
New Patient Registration
We welcome the registration of any new patients residing in our area. You will be asked to complete
a simple registration form. Forms can be downloaded from the above "Registering Online" tab.
You will be registered on the list of a particular GP, however, you are welcome to seek medical
advice from any GP or nurse of your choice. In the interest of consistency and continuity of care we
would ask that, where possible, you consult with the same person for on-going issues.
All new patients are offered a health check with a member of the healthcare team to ensure that any
required tests are up to date and that we have an accurate note of any repeat medication you may be
taking.
Medical treatment is available from the date of registration. Please contact reception for further
information.

Registering Online
Online 'Pre-Registration' With The Practice
If you wish to pre-register click on the link below to open the form. When you have completed all of
the details, click on the "Send" button to mail your form to us. When you visit the surgery for the
first time you will be asked to sign the form to confirm that the details are correct.
Pre-registration Form
When you register you will also be asked to fill out a medical questionnaire. This is because it can
take a considerable time for us to receive your medical records. There is an online version of this file
too, which you may fill out and send to us. When you come to the surgery you will be asked to sign
this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the
Internet and although every effort is made to keep this information secure, no guarantee can be
offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first
visit to the practice.

Temporary Residents
Temporary Registration
If you are ill while away from home or if you are not registered with a doctor but need to see one
you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will
need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the
local practice list and still remain a patient of your permanent GP. After three months you will have
to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not
have to accept you as a temporary patient although they do have an obligation to offer emergency
treatment. You cannot register as a temporary patient at a practice in the town or area where you are
already registered.

details' form and hand it in to Reception.

Confidentiality & Medical Records
The practice complies with data protection and access to medical records legislation. Identifiable
information about you will be shared with others in the following circumstances:



To provide further medical treatment for you e.g. from district nurses and hospital services.
To help you get other services e.g. from the social work department. This requires your
consent.
When we have a duty to others e.g. in child protection cases anonymised patient information
will also be used at local and national level to help the Health Board and Government plan
services e.g. for diabetic care.
If you do not wish anonymous information about you to be used in such a way, please let us know.
Reception and administration staff require access to your medical records in order to do their jobs.
These members of staff are bound by the same rules of confidentiality as the medical staff.
Freedom of Information
Information about the General Practioners and the practice required for disclosure under this act can
be made available to the public. All requests for such information should be made to the practice
manager.
Access to Records
In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may
request to see their medical records. Such requests should be made through the practice manager and
may be subject to an administration charge. No information will be released without the patient
consent unless we are legally obliged to do so.
Complaints
We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a
genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and
as amicably, as possible.

Violence Policy
We ask that you treat your GP and our practice staff with courtesy and respect. We, like many other
organisations in the wider NHS support the governments 'zero tolerance' campaign. Any patient
whose behaviour is considered to be inappropriate, violent or offensive may be removed from the
practice list.

Healthcare Assistants
Healthcare assistants support practice nurses with their daily work and
carry out tasks such as phlebotomy (drawing blood), blood pressure
measurement and new patient checks. They may act as a chaperone when a
patient or doctor requests one.

Donna is responsible for the administration and organisation of the practice. She is
available for discussion of any helpful suggestions or comments regarding service
provision and is also the point of contact for any problems you may have including
complaints.
Reception
Our Reception team is led by Ann Mabbutt. The receptionists are here to help you. It
is sometimes necessary to ask for personal information or details of your illness. This
is simply to ensure that your problems are dealt with as efficiently and appropriately as
possible.
Community Midwives
The midwives will provide care and advice during the antenatal and postnatal periods.
Antenatal clinics are held regularly at the surgery. Please make an appointment to see
your doctor if you believe you are pregnant. Contact details for the midwife will be
provided in your maternity pack; however, if you encounter difficulty contacting the
midwife, you may also do so via the surgery reception during our opening hours.
Community Nurses
The community nursing team consists of a group of qualified nurses working in a
locally based team. They provide skilled nursing services for people in the community
in liason with local hospitals. The service is available seven days a week during normal
hours. An out of hours service is available by phoning 01256 810683 where a recorded
message will tell you how to access this service.
Health Visitors
A health visitor is a registered nurse who has received training particularly related to
babies, children and pregnant women. Their role is to provide families with children
under five years old with support and advice around the general aspects of mental,
physical and social wellbeing.
Through the Child Health Surveillance programme, they are able to detect
developmental and health problems early and offer advice or referral to the appropriate
service. The team has expertise on behavioural problems and sleep management.
Regular child health clinics, postnatal classes and a support group for women with
postnatal depression are held. The health visitors can be contacted on 01329 835303.
