PLANNING IN SILICO SCREENING AND EXPERIMENTAL STUDY OF THE HYPOGLYCEMIC DERIVATIVES OF
CYCLIC GUANIDINES
This article is devoted to the search for new cyclic guanidine derivatives with hypoglycemic activity by means of in silico
methods. The results of computer prediction of hypoglycemic effect for nine classes of these compounds were analyzed.
Two perspective classes of cyclic guanidine derivatives have been identified. Planning in silico screening was performed
and new unstudied substances were tested.

EXCHANGE BILIRUBIN, BLOOD LIPID PROFILE AND ANTIOXIDANT STATUS IN MEN WITH CORONARY HEART
DISEASE
The purpose of this paper – to investigate the relationship of low levels of bilirubin (BR) with lipid profile and blood
antioxidant status in men with coronary heart disease. Our research has shown that low levels of bilirubin is associated
with atherogenic changes in lipid profile and decrease its antioxidant activity. In clinical practice, bilirubin can be used as
an indicator of antioxidant status of the organism.

THE ANALISIS OF THE STANDARDS FOR DECISION OF SURGICAL TREATMENT PROXIMAL HUMERAL.
ANATOMICAL RESEARCH
The article provides information about habits of the routes to the proximal humeral for people with different constitution.
The clinical studies were made on 55 corpses, which were put in order of constitution, body mass and shoulder-elbow
length. Results of studies have shown us that for people with normal body mass better to use anterior approach in a case
of fracture with breaking of the proximal humerus, transdeltoid rout for people with transvers fractures of collum and low
bone mineral density. As for people with subnormal body an anterior approach is more convenient than transdeltoid rout,
and for patients with overweight and lipotrophy of the I degree selection of the rout depends on character of the fracture
and optimal method of the osteosynthesis.

CHANGE A FACIAL FEATURE IN ORTHODONTIC REHABILITATION OF CHILDREN WITH DISABILITIES THROUGHSIDED CLEFT LIP AND PALATE DURING THE MIXED DENTITION
Direct stimulation of the growth potential of orthodontic upper jaw during all periods of the formation of occlusion in
children with unilateral cross-cutting cleft lip and palate makes it possible to restore the size and shape of the dental
arches during the mixed dentition and to ensure stability of the results of complex treatment. Multi-faceted approach to
the rehabilitation of children with orthodontic cleft lip and palate considering to reduce dentoalveolar deformations due to
congenital anomalies and surgical interventions.

CORRELATION BETWEEN THE LEVEL OF STRESS RESISTANCE AND TESTOSTERONE CONCENTRATION IN A
HEALTHY MALES BLOOD
Actual problems is correlation between the level of stress resistance and testosterone concentration in a blood was set:
the more testosterone within normal limits, the higher is level of stress resistance. In 42 healthy men aged 25–40 years
was identified testosterone concentration in a blood by metods electrochemiluminescence. The level of stress resistance
was assessed by changes regulatory-adaptive status. The last was identified by filter samples cardio-respiratory
synchronism in the initial state and the action of stress factor (arithmetic test score under time). The subject with an
excellent regulatory-adaptive ability of the body and a decrease of 5–6% or less in their regulatory-adaptive state were
included in the high level of stress resistance. The subjects with a 50% decrease in index regulatory-adaptive state in
response to stress were included in the moderate stress resistance group. A decrease in index regulatory-adaptive state
by more than 50% was the evidence of low stress resistance. It was that, hearing level stress resistance – testosterone
concentration in a blood 8,38±0,12 ng/ml; moderate level stress resistance – testosterone concentration in a blood
5,73±0,04 ng/ml; low level stress resistance – testosterone concentration in a blood 3,64±0,04 ng/ml. The evaluation of
stress according to previously conducted analysis of the testosterone concentration at inspection allows us to
characterize the willingness of men to stress tolerance was shown and it may be included in the set of tests to recruit
people in extreme professions.

THE APPLICATION ANTIMICROBIAL AGENTS FOR TREATMENT AND FILLING OF ROOT CANALS OF TEETH WITH
PERIODONTITIS
The article presents the results of applying a combination of two antimicrobial agents for medical treatment and filling of
root canals of teeth with periodontitis. A study of the nature of the microflora of root canals in 128 patients aged 25 to 54
years. In the treatment of periodontitis used a combination of trimethoprim/sulfamethoxazole (TMP/QMS) and its
combination with metronidazole. Analysis of clinical observations have shown that the use of TDM/QMS and its
combination with metronidazole in the treatment of periodontitis has a positive effect for 88,7% of patients.

THE BASIC MODERN PRINCIPLES AND APPROACHES TO THERAPY OF THE ENDOGENOUS PSYCHOSES WITH
THE FIRST ATTACK AT YOUTHFUL AGE
It was carried out detailed clinical and follow-up examination of 347 patients of youthful age (16–25 years), with the
episodic type of schizophrenia (F20 in ICD-10), hospitalized with manifest psychotic attacks in clinic of MHRC of the
RAMS during the period from 2000 to 2013.
A number of the key provisions, allowing to structure approaches to the organization and carrying out therapeutic actions
at patients with first attacks of the youthful schizophrenia was formulated. The main tactics of purpose of antipsychotic
therapy at various stages of a course of schizophrenia was described, besides, need of use of methods of mental
correction and social rehabilitation as a part of complex treatment of patients of youthful age was proved.

INTEROPERATION OF RESEARCH OF GASTRODUODENAL TRANSITION AFTER TAKING IN OF PERFORATIV
DUODENAL ULCER
In article results intraoperational researches sewed up perforation ulcers are described, the basic pathoanatomical
structures above a stenosis parts of a duodenal gut and gastroduodenal transition are designated. The received data
about pathomorphology sewed up ulcers testify to their huge morphological variety, necessity of an individual approach
for surgical treatment sewed up perforation ulcers.

THE INFLUENCE OF BACTERIAL VAGINOSIS DURING PREGNANCY, THE FETUS AND NEWBORN

ESTIMATION OF FUNCTIONAL ADAPTATION OF THE TRANSPLANTED HEART
IN AN ORGANISM OF THE
RECIPIENT
Significant development of the heart transplantation in clinical practice allowed implementing these types of surgery in most
of the major cardiac clinics in the world. This happened because of the improvement of surgical techniques, artificial blood-
circulation apparatuses and especially immunosuppression methods. It goes without saying, that these achievements raise
the new questions. One of the most important one among them is the recovery of extracardiac regulation, especially the
nerve regulatory effect on the transplanted heart. Six months after surgery we’ve examined 10 patients. Cardiorespiratory
synchronism was not detected among all the patients, primary pacemaker in the sinoatrial region of the heart showed no
reaction to the Danini-Aschner reflex. These facts showed no signs of the heart’s functional reinnervation and recovery of
mechanisms of the heart rhythmogenesis hierarchical organization. At the same time, changes in heart rate variability of
these patients gave us the reason to assume a manifestation of the initial stages of incorporating the central nervous system
influences to the heart rate. The spectral analysis of heart rate variability among these patients showed that TP ms2 they
amounted 152,45 ± 13,48; HF ms2 – 59,85 ± 25,02 and VLF ms2 – 74,55 ± 33,16.

STATISTICAL FORECASTING WHEN CREATING A QUICK TECHNIQUE OF DETERMINING A LEVEL OF
REGULATORY AND ADAPTIVE STATUS OF THE HUMAN BODY
In order to improve the existing incremental principle of determining the cardiac and respiratory rhythm synchronization
(CRS) range the ‘quick technique’ of determining the index of regulatory-adaptive status (IRAS) of the human body has
been proposed. In order to reduce the number of samples we proposed to apply the statistical forecast of regulatory and
adaptive status (RAS) (from ‘high’ to ‘low’) on the basis of its least-consuming measurements or by fixing the CRS
parameters. The following factors/arguments have been examined as the basis of the forecast: the minimum boundary of
the synchronization range, duration of evolution synchronization at the minimum boundary, the following categories also
have been taken into account: initial respiratory rate, initial heart rate, age, height, weight, gender, menstrual cycle phase,
diastolic and systolic blood pressure values.
Linear discriminant analysis and neural network (NN) classification techniques – multilayer perceptron and network of radial
basis function type have been analyzed. Neural network classification has shown the best accuracy. Assessment of
accuracy and confidence level of the forecast has been performed. On the basis of the NN-classification RAS level has been
proposed curtailed incremental definition of IRAS technique: possible to complete the examination in 1 stage (probability
70%), 2 stages (probability 90%), or 3 (control) stages (samples), it is depending on the subject’s observed RAS level.

FEATURES OF DISORDERS OF REGULATION MECHANISMS AT THE ORGAN LEVEL OF THE CIRCULATORY
SYSTEM IN HYPERTENSIVE PATIENTS IN TERMS OF COMORBIDITY WITH RHEUMATOID ARTHRITIS
This study analyses the changes of the cardiovascular system organs of 254 patients with stage 2 hypertension, 1–2
grade combined with rheumatoid arthritis (RA) in the initial state. After screening and obtaining written informed consent,
the patients were divided into two groups: group 1 consisted of 142 patients in the stage of remission with both clinical
and laboratory features, group 2 included 112 patients with severe symptoms (flare-ups). Myocardial structural changes
exhibited by both groups of patients correspond with the common development of left ventricular (LV) hypertrophy, the
increase in absolute and relative values of the morphometric parameters. Identified correlation confirms the negative
impact of the degree of activity of RA on renal function, indicating a higher risk of cardiovascular complications in this
group of patients.

PERIODONTITIS PAIN SYMDROME INFLUENCE ON REGULATIRY-ADAPTIVE CAPACITY OF HUMAN ORGANISMS
30 patients with periodontitis have been involved in identification of index of regulatory-adaptive capacity status prior to
treatment and after it, and from it they have determined regulatory-adaptive capacity of organisms. The more severe pain
was the less regulatory-adaptive capacity of human body was. Regulatory-adaptive capacity has increased after remove
of pain by treatment of periodontitis. Consequently, regulatory-adaptive capacity of organism can serve as an index for
periodintitis treatment efficiency.

EVALUATION OF BIOCHEMICAL AND HISTOLOGICAL INDICES OF THE PULP INTACTIX TEETH IN THE
EXPERIMENTAL CONDITIONS PULPHYPERTHERMIA
The article presents the results of experimental studies on the activity of acid lysosomal field of glycosidase inhibition (βglucuronidase, β-glucosidase and β-N-acetylglucosaminidase) and their influence on the function of cells of the pulp in
the conditions pulphyperthermia. Experimental study was conducted in 6 mongrel dogs. It is established that the
temperature factor odontophoridae influences the activity of the acidic lysosomal field of glycosidase inhibition,
significantly worsening the conditions for the functioning of the cells of the pulp of the tooth, which necessitates the use of
activators zingiberone field of glycosidase inhibition and inhibitors aktivirovat field of glycosidase inhibition of dental pulp
after odontophoridae.

EVALUATING THE EFFECTIVENESS OF THERAPY AND REHABILITATION OF PATIENTS WITH CHRONIC
GINGIVITIS IN THE APPLICATION OF ESSENTIAL OIL
The article presents an assessment of the effectiveness of the use of tea tree essential oil in therapy of patients with
chronic gingivitis. The study involved 43 people who underwent clinical and laboratory examination. It has been
determined that the use of tea tree essential oil in the treatment of patients with chronic gingivitis reduces the duration of
therapy, the effectiveness of the rehabilitation of patients, which was significantly confirmed by the data as a clinical and
laboratory examinations.

EFFECT INSUFFICIENT OF MAGNESIUM ON QUALITY OF LIFE IN YOUNG PATIENTS WITH UNDIFFERENTIATED
CONNECTIVE TISSUE DYSPLASIA
Surveyed 29 people aged 20–25 years, 19 of which have a insufficient of magnesium, osteoarticular symptoms
undifferentiated dysplasia оf connective tissue, who have been diagnosed with insufficient of magnesium and 10
practically healthy persons. All surveyed were divided into 2 groups: the first group consisted of 19 people, and the
second 10. Evaluation of quality of life (QOL) and its comparison groups. The marked decline in the quality of life in
the group 1 compared to group 2 on a range of indicators (role functioning, pain, general health, vitality, physical
health component).

PROGNOSTIC SIGNIFICANCE OF GENETIC FACTORS IN THE DEVELOPMENT OF PROLIFERATIVE DISEASES OF
THE UTERUS
Were studied reproductive health of 200 women aged from 18 to 48 years, of which 64 with fibroids, 33 – with fibroids in
combination with adenomyosis and 103 – without fibroids and adenomyosis. Performed genotyping of six molecular
genetic markers (SNPs-351A/G and -397 T/S ESRα gene, a polymorphic marker genes of tumor necrosis factor α
receptor tumor necrosis factor 1-St and 2-nd type and lymphotoxin α) to study the role of family history in the formation of
fibroids in the uterus in women. Women with genotypes GG, GA, AA polymorphism +36 A/G receptor tumor necrosis
factor 1 and type GG polymorphism +250 G/A lymphotoxin α are increased risk of developing uterine fibroids and
adenomyosis.

COMPARATIVE EVALUATION OF THE DECREASE OF VISUAL FUNCTIONS AND CHANGES OF MORPHOMETRIC
PARAMETERS OF MACULAR AREA WITH THE PATIENTS WITH POST-THROMBOTIC MACULAR EDEMA
In this work comparative evaluation between the degree of the decrease of visual acuity and the data of morphometric
research of the macular area (thickness of the central area of retina and the volume of the macular area) with the patients
with macular edema developed against the backdrop of thrombosis of top temple branch of the central vein of retina was
made. The data received make it possible to find the tactics of the future treatment and prophylaxis therapy of the
patients with post-thrombotic retinopathy.

RESULTS OF RADICAL SURGERIES AT THE COMPLICATED COLORECTAL CANCER
Surgical treatment is carried out 1998 patients with urgently complications of a cancer of a thick gut which during the
period from 2001 to 2011 were treated in hospitals of Smolensk and the Smolensk region. Intestinal obstruction was
observed in 1644 cases (82,3%), perforation of a tumor 208 (10,4%), bleeding of 146 (7,3%) (n = 146). From among all
operated patients radical operations are executed at patients 1427 (71,4%). Radical surgery was carried out by the
patient of advanced and senile age less than at persons of younger age. At men with the complicated cancer of a thick
gut the bigger percent of radical operations in comparison with women is noted. Besides, the volume, and nature of
operation depended on hospitalization terms from the moment of development of complication, weight of the general
state, prevalence of malignant process and existence of the remote metastases.

MENTAL DISORDERS IN ENDOCRINOPATHIES
This article presents a literature review of today's challenging clinical problem of mental disorders in endocrinopathy. After
an introduction to the topic describes a brief history of the development of psychoendocrinology and putative mechanisms
of the pathogenesis of mental disorders caused by endocrine dysfunction. Outlines the main clinical forms of mental
disorders, as well as analyzes the patterns of their clinical dynamics. Marked clinical manifestations caused by a nonspecific impact of endocrine dysfunction, as well as the specific features of each individual endocrinopathy. In conclusion,
describes an integrated approach for the treatment and rehabilitation of patients with these forms of pathology. Given the
prevalence of endocrinopathies and frequent combinations thereof with mental disorders information on this issue is
needed for professional doctors of all specialties.

REGULATOR ADAPTIVE POSSIBILITIES IN PATIENTS WITH GENERALIZED ANXIETY DISORDER
The article represents the study results of regular adaptive possibilities in patients with (generalized anxiety disorder)
GAD. The patients were divided into 3 groups according to the Hamilton anxiety scale. Patients with mild and moderate
levels of anxiety had satisfactory regular adaptive possibilities, and bad possibilities in case of severe anxiety.
Assessment of regular adaptive status reflects the severity of GAD.

CYTOKINE INDICATOR OF THE ORAL CAVITY – A NEW INDEX IN THE DIAGNOSIS OF DENTAL DISEASES
On the basis of the study, 20 patients with chronic generalized periodontitis of moderate severity, 20 patients with single
fractures in the body of the mandible and 20 volunteers with intact periodontium developed a new immunological indicator
to differentiate between acute dental process from chronic and very dynamics of transformation, which adequately
reflects the seriousness of the disease and effectiveness of treatment.
