(Independent contractor position-15 hours per week, evening and weekend hours)
Reports To:
1.) US Referrals Program Director 2.) Patient Care Coordinator
Purpose and Scope of Job:
Assists professional staff by providing auxiliary services for pediatric patients and their guardians such as case
management assistance, transportation to medical appointments, assistance with personal errands and language
translation assistance where necessary.
Essential Functions (other duties may be assigned):

from full time case management staff, the case aide will:
immediate patient/guardian needs as they arise:
Assist with late hospital discharges.
Relieve full time program staff and cover after hour medical appointments.
Visit guardians and children during their hospitalization
Pick up and deliver prescription medications to families and ensure understanding of the
medication instructions.
2. Meet social/group living needs at group housing facilities:
a. Provide assistance and visit with families staying at the partner housing facilities.
b. Provide assistance to families for daily living activities.
c. Facilitate grocery shopping trips and other personal errands as needed.
3. Foster positive family environment:
a. Provide activities and group interactions to build positive relationships between families and add
structure to down time (games, crafts, educational activities, group activities).
4. Maintain frequent oral and written communication with U.S. Referrals team for patient updates and
continuity of case management. Remain flexible to respond to ever-changing priorities.
Non-Essential Functions (performed on an infrequent basis):

Transport families to meetings and/or special events
Occasional office tasks pertaining to patient needs
Minimum Qualifications:
High school diploma or equivalent, associates or bachelor’s degree preferred, clean driving record and access to a
vehicle with valid insurance. Must be able to pass criminal background check and driving record inquiry. Must be
fluent in written and oral English and Spanish.

Excellent communication skills to produce clear and concise written and oral reports
Ability to work with families from diverse cultures and backgrounds.
Experience working/volunteering in following areas is preferred: child/family services, medical field, nonprofit/social service agency
Ability to establish rapport and provide a safe and calm environment for children and families
Able to respond to the needs of a culturally diverse client base, assess possible problems and take
appropriate actions
Work cooperatively with staff, supervisors and families
Maintain patient/client privacy standards.

Must be comfortable working with varying ages of children (newborn-21) and comfortable with various
medical diagnoses. Must be able to maintain professional boundaries when working with families.
Interactions with Others (Internal and External):
Internal: Families (children and guardians), staff and volunteers.
External: Group housing staff/volunteers, medical and non-medical hospital staff and social workers.
