feel more comfortable, please let us know. Enclosed is a brief introduction
about this dental office and some forms our office requires that you read
and complete before treatment. If you have any questions please ask, we are

About Your Doctor
Dr. Curtis J. Perry is an Oakland native, residing in Oakland with his wife
and two children. Attended Bishop OʼDowd High School, received his B.S.
in Biochemistry from Cal-State University Hayward and Doctorate of Dental Surgery from University of California, San Francisco Medical Center in
1983. He enjoys playing jazz/R&B bass guitar, computing, skiing & scuba
diving. As an active member of local dental societies including Alameda
County Dental Society, Greater Bay Area Dental Society, California Dental
Society, American Dental Association, Dental Organization for Conscious
Sedation and the American Dental Society of Anesthesiology. He stays current with the dental community at large, new techniques, while continually taking many cosmetic, orthognathic, anesthesia and general dentistry
courses.
New Patient
First visit usually will include a radiographic survey. Full mouth x-rays need
to be taken every 3-5 years and checkup x-rays taken every 6-24 months. If
a current set is available bring or have them sent ahead of your visit. We
will also need information about your insurance coverage. Should any of
this information change between visits, let us know so we can update your
records.

Appointments
We realize your time is valuable so appointment time is reserved for you.
If your appointment time is affected by an office emergency we will call
you to inform you and make changes if necessary. To avoid a charge
please give two dayʼs notice if you must cancel an appointment.
Office parking is usually available on 32nd Street or Telegraph Avenue.
On 32nd Street there is 2 hour on the near side and indefinite on the far
side; Metered parking on Telegraph Avenue.
Variety of Services
Most services are available to you at this office. Cosmetic dentistry including tooth bleaching, white fillings, bonding, tooth colored crowns,
bridges and porcelain veneers. Comprehensive service is offered to
treat cavities and missing teeth, with gentle and high quality care. Fillings (gold, silver, composite or porcelain), simple extractions, partial and
full dentures, and implants. IV sedation or oral sedative medications and
laughing gas are available to help relax the anxious patient. We take all
precautions to ensure your safety.
Periodontal Health
We emphasize periodic preventive checkups to keep your dental health
stellar. Periodontal Health is of utmost importance for your teeth to last
your life. For adults who have not had regular dental visits, it is not unusual for us to detect gum disease. If you have a problem we will prescribe a treatment plan to address your particular needs. Please understand that if your situation is serious a simple cleaning may not remedy
your problem. You may need to be seen by a specialist for treatment. We
will be looking out for your long-term health and work to prevent future
tooth loss or problems.
