The Guide includes information on more than 160 vendor organizations. Moving forward, we hope to continue to supplement our own research with feedback from both vendors and clinical end-users alike. To help organize this mountain of data, we have included bcc: Consulting’s framework for segmenting the mobile health market. We have found this structure to be a clear and concise way to quickly understand the competitive landscape, as well as to identify solution categories and companies offering products within them. Our goal was to make the segmentation as intuitive as possible. Nevertheless, we recognize that it is not perfect. To help further explain the structure and rationale of our segmentation, we discuss four general sections of the Master table heading. To begin with, each mobile application category is arranged along a continuum from those focused purely on the inpatient setting on the left side to those focused purely on the outpatient on the right. However, the ‘Inpatient Foundation’ group is not limited to Rounding. Inpatient Orders and Inpatient Notes have become standard, dependent complements to Rounding. As a result, we view these three, typically bundled, inpatient application categories as the basis of any hospital’s long-term mobile plan. II Other Inpatient Applications ‘Other Inpatient Applications’ are those that are not part of the ‘Inpatient Foundation’ group. Currently, the most common applications in this area support non-physician hospital staff such as nurses and pharmacists. Emerging, purely-inpatient focused, applications will likely develop at the department level, as various interest groups leverage the ‘Inpatient Foundation’ to serve their own unique needs. III The Overlap While there is a significant overlap between the inpatient and outpatient applications — following typical clinical workflow patterns — the amount or degree of applicability varies from solution type to solution type. We have arranged the application categories in this area by the relative applicability to each clinical setting. The color gradient across the top of the section approximately indicates the relative proportion of inpatient/outpatient focus for each application type. For example, Charge Capture is primarily an inpatient application (refer to Going Mobile for a more detailed explanation), while e-Prescription is almost purely an outpatient solution. 10 w w w . h i t b c c . c o m Copyright © 2006. Most vendors offering solutions within this area are focused on one specific solution type. Some may offer multiple, such as Charge Capture, e-Prescription, and Dictation Capture, but these vendors commonly sell the products independently. These solution types can also be viewed as ‘add-ons’ to either ‘Inpatient’ or ‘Outpatient Foundation’ packages. IV Outpatient Foundation Similar to Inpatient Foundation, the application types within Outpatient Foundation are linked by a common dependency. Specifically, in the case of Inpatient Foundation, both Inpatient Notes and Inpatient Orders require the existence of Rounding — for all purposes a quasi-inpatient EMR. In the case of Outpatient Foundation, Scheduling, Outpatient Notes, Outpatient Orders and Outpatient Results all depend on direct access to a clinic or practice group’s PMS and/or EMR. It is not surprising then that most companies offering solutions within the Outpatient Foundation group offer most, if not all, of the application types. Typically these companies own the proprietary EMR that they support and the mobile extension is really just an extension to the EMR. Unlike the inpatient setting, most PMS and EMR systems remain closed systems. Consequently, any given practice group is likely to be limited to its source vendor for ‘Outpatient Foundation’ functionality. 11 w w w . h i t b c c . c o m Copyright © 2006. For terms and conditions of use, contact bcc: Consulting. bcc: Mobile Resource Guide • Q1 06 Mobile Health Applications Rounding A Rounding solution helps physicians manage their census information, as well as associated clinical results data (including lab, pharmacy, and transcribed reports). Some Rounding solutions support diagnostic images, as well as other information such as vitals and ins/outs, depending on availability within the back-end hospital information systems. Rounding solutions help clinicians answer three basic questions: 1. Who are my patients? 2. Where are they are located? 3. What are their test results? Additionally, Rounding solutions should provide advanced census management to help physicians in practice groups or working in team environments to easily identify and manage a more comprehensive set of censuses, thus simplifying cross-coverage and call duties. These solutions typically save clinicians anywhere from 30 to 90 minutes per day, depending on inpatient census load. Beyond the time saved, Rounding solutions claim to improve the quality of care, and potentially reduce length-of-stay, by making critical data more readily accessible when and where it is needed. 12 w w w . h i t b c c . c o m Copyright © 2006. For terms and conditions of use, contact bcc: Consulting. bcc: Mobile Resource Guide • Q1 06 Inpatient Orders ‘Inpatient Orders’ is a fairly vague description of fairly vague functionality. In theory, Inpatient Orders should refer to full-blown order-entry. Given the challenges of deploying computerized physician order entry (CPOE) on a 17-inch monitor, though, most self-aware inpatient mobile solution providers approach this topic very cautiously. It would be foolish, at least in the near-future, to discuss full order-entry on a handheld device. As a result, most vendors are taking an incremental approach. The first Inpatient Orders functionality that is only now being deployed is the ability to view the status of all orders with, potentially, the ability to co-sign preliminary orders. Additionally, most vendors offering orders functionality are including support for a select few high-value lab orders. The next step will likely include more sophisticated physician ‘hotlists’ of common orders, including standard diagnosis related order-sets. Inpatient Notes ‘Inpatient Notes’ specifically refers to using a PDA to handwrite notes and other documentation during an inpatient encounter in an ad-hoc format. Handwritten notes captured in this format are usually maintained on the PDA and/or intermediate staging database, and are thus available for viewing during the patient’s length of stay. This information, however, is generally not incorporated into transcribed reports or maintained within a patient’s record. Additionally, solutions that maintain notes on an intermediate staging database may be shared with other caregivers who have access to the same census. Other Nursing Any solution, other than e-MAR support, where the chief beneficiary is the nursing staff, typically within the inpatient environment. There is not yet an industry-standard for ‘Nursing’ functionality. The most common features are data capture (i.e. vitals, ins/outs, and other diagnostic data), care notes documentation, remote monitoring, and orders administration. e-MAR Many hospitals have implemented electronic medication administration record (eMAR) systems from companies such as Pyxis, Bridge Medical, Omnicell, as well as the major HIS vendors. e-MAR systems help automate medication administration to ensure the 5 rights: (1) right patient, (2) right medication, (3) right time, (4) right dosage, and (5) right route. While many of these system providers offer some level of handheld functionality, currently e-MAR systems and ‘mobile’ solutions are two distinctly different solutions. Nevertheless, moving forward, look for mobile solution providers to implement at least limited support for e-MAR systems, either accessing and displaying data from an existing 3rd party e-MAR or attempting to replicate some of an e-MAR’s functionality. Pharmacy ‘Pharmacy’ solutions assist hospital-based pharmacists with day-to-day rounds and documentation. For terms and conditions of use, contact bcc: Consulting. bcc: Mobile Resource Guide • Q1 06 Charge Capture Charge Capture applications exist to help physicians more accurately document patient encounters for billing purposes. On the surface, Charge Capture would appear to be an outpatient application, as the bottom line effect and value-add of such solutions should be higher reimbursement levels as well as better documentation to support claims—largely private physician and physician group benefits. Nevertheless, the opportunity for Charge Capture exists in the inpatient market, far more so than in the outpatient. First of all, documenting encounter data (procedure and diagnoses codes) in the outpatient setting is typically not a ‘huge’ problem. Practice groups have long since developed effective means of documenting encounters, including utilizing pre-printed superbills or encounter forms. Furthermore, while in their offices, physicians work in a very controlled environment. Procedures are typically scheduled. Physicians have constant access to their support staff, and ultimately operate within very refined processes. Very few physicians, in only select circumstances, would invest the time and energy to learn a new system to replace one that works. The problem for practice groups occurs when physicians leave their office to go to the hospital. Things start to break down. Depending on their specialty, chances are far more likely that a physician will be performing a greater number of procedure types, or at least ones that are less familiar. Oftentimes these outlier procedures represent higher dollar reimbursements. Second, physicians do not have direct access to their office staff, and therefore are dependent on their own diligence for documenting the encounter and ensuring the data makes it back to their office-based billing person. As a result, codes are frequently jotted down on loose pieces of paper, the back of hands, scrub pant legs, or any number of other creative, makeshift notepads. Third, if encounter data actually makes it back to the office ‘coder’, there will inevitably be issues concerning the codes’ correctness, and the specific procedure(s) performed, as well as appropriate diagnoses. Needless to say, claims data that is adjusted days later, based largely on a physician’s memory, is problematic at best. Because of the challenges in reliably capturing quality data, Charge Capture fills a very important, high-value function for predominantly independent physicians when they enter the hospital. Dictation/Transcription Dictation/Transcription specifically refers to using a PDA to record dictated notes which are then transferred to a transcription solution or service. Most new PDA devices natively support voice recording functionality. Dictation Capture solutions leverage device recording functionality and incorporate additional advanced features, including encryption, integrated census data, diagnosis/procedure specific templates or prompts to help ensure that physicians are capturing all the relevant information, and more. Additionally, several Dictation Capture solutions now support the delivery of preliminary transcribed reports for physician review and e-signature. 14 w w w . h i t b c c . c o m Copyright © 2006. For terms and conditions of use, contact bcc: Consulting. bcc: Mobile Resource Guide • Q1 06 Beyond streamlining clinical workflow, Dictation Capture can significantly reduce transcription costs to hospitals. Transcription services typically charge 10-15 cents per transcribed line. By integrating electronic demographic data into voice file headers, Dictation Capture solutions can typically save a few lines of transcription per report. Furthermore, by tagging each file with appropriate patient name and identifiers, Dictation Capture greatly reduces the likelihood of error when coupling the final report to the correct medical record. Content/Decision Support Content refers to applications that provide medical reference material on a handheld or mobile device. The material is typically static, and often an exact replication of popular, traditional printed medical reference material (such as Griffith’s 5 Minute Clinical Consult, The Physician’s Desktop Reference, and many others). Over the last few years, many of the publishing houses, as well as their third party distributors, have realized the value of customizing traditional print material to better exploit the dynamics of mobile devices. Decision Support also refers to medical reference content. The distinction is that Decision Support applications possess a dynamic element—such as a decision tree or algorithm—and generally incorporate the latest best-practices within a specific area ordisease state. For example, TheraDoc’s Antibiotic Assistant provides patient-specific and disease-specific anti-microbial recommendations based on a set of contextbased input data. Given the size of knowledge bases, and the potential number of input variables, very few true Decision Support solutions can be implemented on today’s handheld devices. As PDA-based content has expanded its functionality to include keyword searches and other automated indexing, the lines between static and dynamic content have become blurred. Lexi-Comp, Micromedex, Skyscape, and Epocrates, for instance, all offer APIs to support 3rd party software interoperability. The result can provide a very dynamic, patient-contextualized aspect to otherwise static content. Medical Devices Medical Devices solutions connect external sensing hardware to PDAs or other mobile computing devices via popular, native interfaces (PCMCIA, MMC, USB, etc.). The resulting systems are used as diagnostic devices, or ‘mobile’ medical devices. The value of these systems over traditional medical devices is decreased cost, and increased portability of both the device and the data collected. Traditional ‘standalone’ medical devices have been ubiquitous in healthcare for many years, but with only limited integration with other clinical systems. As EMRs and other electronic health records have grown in popularity, so too has the need for integrating the results from diagnostic devices. ‘Mobile’ medical devices benefit from being built on mobile consumer devices, such as PDAs, which offer a large number of connectivity methods. A single PDA might offer Bluetooth, WiFi, IR, MMC, CF, USB, and serial connections. One of the core values of ‘mobile’ medical devices is the potential to use those connections for moving data into common electronic records. 15 w w w . h i t b c c . c o m Copyright © 2006. For terms and conditions of use, contact bcc: Consulting. bcc: Mobile Resource Guide • Q1 06 Clinical Trials Clinical Trials applications assist in the capture and aggregation of pertinent data during a clinical trial. PDAs and mobile devices are most commonly used to support electronic patient diaries, however, mobile devices may also be used in tandem with other hardware to function as medical devices (see below). Companies offering mobile clinical trials applications target the historically difficult, time-consuming, and flawed process of data gathering in the hopes of simplifying and streamlining costly clinical trials. By improving both the quality and quantity (sample size) of data, clinical trials applications possess the potential to cut months and even years off of the regulatory approval process. This can potentially save millions of dollars for pharmaceutical, biotechnology, and medical device companies. e-Rx e-Rx, or electronic prescription, refers to the use of PDAs and other mobile devices to assist in the drug prescription process. While ‘e-Rx’ is often associated with an end-to-end electronic process for creating and delivering a drug prescription, it is important to note that e-Rx is not an all or nothing solution. The prescription writing process is exactly that, a process. The process has multiple steps, with each step representing the opportunity for both efficiency and safety improvements. As a result, there is a continuum of potential. The easiest ‘e-Rx’ solutions to implement, and the least expensive, deliver up-todate drug reference material to assist physicians in understanding pertinent drug information (drug name, dosage strengths, contraindications, alternatives and directions), such as Epocrates’ Rx. These ‘supporting’ solutions (they do not actually generate scripts) may also provide dose calculators, as well as general and patientspecific formulary information. More involved ‘e-Rx’ solutions enable physicians to create prescriptions on a standalone basis. These solutions are more commonly referred to as prescription ‘writers’ and while they may support access to schedule information from a practice management system, including demographic data, as well as third party payer and specific formulary information, they do not support access to long-term drug history, active medications, and other key clinical data. The resulting script typically must be printed off on a local printer and either faxed to the pharmacy, or hand-delivered by the patient. Prescription writing solutions add value in multiple areas, including eliminating legibility issues. Standalone prescription writing solutions are typically more expensive, and difficult to implement as they require physicians to modify their workflow. Full-blown e-Rx solutions are the most expensive and difficult to implement. Unlike standalone prescription writers, full-blown e-Rx solutions integrate seamlessly with EMR systems. Physicians should have direct access to full patient information, including known active medications. This information should automatically crosscheck for potential adverse drug events, as well as potential formulary conflicts. Upon creation of the script, a full-blown e-Rx solution will transfer the information to the patient-directed pharmacy electronically. Full-blown e-Rx solutions should further support bi-directional communication between physician and pharmacy to streamline prescription renewals, as well as pharmacy initiated script modifications. 16 w w w . h i t b c c . c o m Copyright © 2006. Mobile ‘Scheduling’ allows doctors to view and possibly edit their schedule from a PDA or other mobile device. Solutions offered may be ‘standalone’ solutions that maintain the schedule on the mobile device, or they may be integrated solutions which interface with a PMS system and maintain the schedule centrally. A clear benefit to integrated solutions is the potential to access the schedule from a variety of locations using a variety of devices. Outpatient Orders Outpatient Orders most commonly refers to ordering labs from a handheld device, however it may extend to other diagnostic tests. This functionality depends on the primary system (such as a clinic’s EMR) to process the orders. Outpatient Notes ‘Notes’ provides physicians the ability to record useful, patient-specific information in an unstructured format, however many vendors are now developing templates to capture information in a structured manner. This information may or may not be incorporated into transcribed reports or maintained within a patient’s record. ‘Outpatient Notes’ functionality may be tied into a physician’s office-based PMS/EMR system, or they may be maintained only temporarily on the handheld device. Outpatient Results ‘Outpatient Results’ refers to the display of test results on handheld devices within the ambulatory environment. Physicians have traditionally relied on fax, mail, and courier-delivered results from labs. New EMR systems have begun to integrate the lab results directly, and mobile ‘outpatient results’ solutions allow the physician to view that information in a handheld format. 17 w w w . h i t b c c . c o m Copyright © 2006. Companies are sorted into the following categories: Inpatient /Outpatient Solutions that were created to span both inpatient and outpatient environments Inpatient Focused Solutions that are typically add-ons to inpatient mobile solutions, and come with little to no PMS integration Outpatient Focused Solutions developed specifically for the outpatient setting (i.e. HL7 integration to HIS systems is not typically included) Standalone /Reference Solutions that are primarily intended as standalone references Companies are then further divided into those systems that are cross-system compatible (Open), and those that are dependent on existing, Proprietary infrastructure (PMS/EMR and HIS). 2. Vocabularies Companies offering charge capture via tablet PC generally offer complete ICD-9, CPT-4 and HCPCS vocabularies. PDA-based applications, once constrained by memory limitations, have begun to incorporate the entire vocabularies on the mobile device itself. Specialty and customized “short-lists” are useful, but can also be frustrating if a physician cannot find the right code when needed. Intelligent search capability at the device level has become increasingly important, and will likely become a differentiating factor. 3. Claims Editing Includes CCI, Global, age, gender, and LMRP edits on the device and/or claims management interface. While some software companies claim to support claims editing, we encourage you to take a close look at the source of edits content. High value-add content is nearly always licensed from 3rd parties specializing in claims editing. 6. Management Interface Web-based and PC-based. Not to be mistaken with ASP support. Companies offering a webbased management interface allow either/both physicians and authorized office staff to remotely log into a web-based program to view encounter data. Encounter information should be viewable, modifiable, and downloadable/exportable. PC-based management interfaces are generally premise-based and limited to specific PCs. 7. Pricing Refer to price tiers. In general, an assumption is made that proprietary charge capture systems first require the purchase of the underlying infrastructure or system (PMS/EMR or HIS). As a result, most proprietary systems are considered costly. If, however, you already have the underlying system, chances are that the proprietary offering is your least expensive alternative. The challenge arises from the vast number of content providers, integrators, and resellers and their often confusing relationships. In assembling this segmentation, we have focused on one thing alone - what a clinician needs to understand when selecting PDA-based content. • To begin with, the companies presented in the top header (under the VAR, Drug Database, and Content Portal tabs) are those who have either intentionally, or unintentionally, placed themselves in a position to be a clinician’s single mobile content platform. VARs Unbound Medicine, Skyscape and Handheldmed have developed extensive relationships with the many content publishers in the hopes of achieving a critical mass of key medical titles. Drug Database These companies, while arguably not having the same relationships with traditional publishers, own the premier drug databases. In other words, while they do not have the volume of content, they own the crown jewel of content in the world of mobility — drug data. Companies like Epocrates have accomplished phenomenal market penetration with Rx and RxPro and have worked hard to expand their services and offerings to be the primary content resource, even if not the only. Content Portal These companies are content publishers who have come to realize the potential of a combined PDA and internet offering. They tend not to have as evolved mobile technology, but generally have the resources to acquire it. Their real strength comes from owning many of the key medical titles. Solutions that bridge the key channels, such as VAR and Content Portal (via Unbound Medicine’s relationships with both McGraw-Hill and Ovid) have the potential to integrate a vast wealth of content resources. • Q1 06 The left hand column details specific technology proficiency, such as PDA OS, and linking capabilities, as well as the sources and types of content offered. Note: Many of the specific content solutions near the bottom are not supported by one of the content platform companies directly. In general, these are standalone solutions that must be obtained directly from the source vendor. Additionally, not all guideline resources and references are included in the above segmentation. For the most part, the above references require purchase. Many other PDA-based guideline tools are available in bcc’s Guide to Free Applications. For clinicians seeking a broad array of content — drug databases, news updates, etextbooks, etc... — it is a near certainty that multiple vendors will be needed.
