General: Prior to rotation, page Dr. John Kula for instructions to begin your rotation. Use the checklist to have
attendings sign as they cover the topics. At the end of the rotation, turn in the checklist to your residency
coordinator so that it can be forwarded to Dr. Kula who will do your composite evaluation. The checklist will be
placed in your portfolio.
