Students who are descendants of a veteran of World War I are eligible to
apply for this scholarship. The application priority deadline is February 15th.
1. On whose service in the Army or Navy of the United States do you base
this application?

3. Give the date and place (including the state) of enlistment whether this
person was in the United States Army, Navy, or Marine Corps.

(Discharge certificate or certified copy must accompany this application.)
5. Submit birth certificates showing the line of descent from the person on
whose service you are basing this application. You must be a direct
descendent.
