6A Medication Chart This Med Chart is to make you aware of any medication’s name, use and side effects you received while at Upstate Hospital. The nurse will highlight the medication you were given. You may not go home on these medicines. The information is presented very simply. Please ask your nurse if you need more information for any medication you get while here or the medications you go home on.
