Chemical Peels
If you are concerned about sun-damaged skin (uneven skin pigmentation and texture)
or acne, then you will be interested in the benefits of chemical peels. Basically, peels
consist of the application of an acidic solution to the skin that promotes the shedding
of the most superficial layer of skin cells. This exposes the smoother, younger-looking
skin below. Some mild pigmentation in the epidermis may be shed in this manner.
Peels are stronger than the daily use of similar products and, therefore, they work more
quickly and effectively. Those individuals with rosacea or facial spider veins must be
treated more gently or the facial redness can be exaggerated.
The Vitalize Peel® is a strong but well-tolerated peel which is excellent for acne and
sun-damaged skin. It has several peeling agents - Retinoic Acid (Retin-A®), salicylic
acid and lactic acid. The skin may look and feel like a mild windburn for the next two
to three days and then the skin will peel off. It is not the flaking that normally comes
with Retin-A® use but more like the peeling of a sunburn.
Once the skin peels, the skin should look clearer and smoother. Noticeable results may
be seen with one peel, but most people will want a series of three to six at three week
intervals.
The AFA Clay-Peel™ and the AFA™ peels are milder peels which also are useful in
the treatment of sun-damaged skin, melasma and acne. These peels are directed
towards people who have more sensitive skin or who don’t want the redness and
actually peeling of the Vitalize™ peels.
AFA’s are Amino acid Filaggrin-based Antioxidants. They work similarly to glycolic
acid in that they are exfoliating but they also help retain moisture in the skin. They
have little irritation. The AFA product line can be used on a daily basis to complement
the effect of the chemical peels.
For more sensitive skin, the AFA peel is performed at a lower concentration without
the clay mask. A mild burning sensation may be felt. Individuals with acne or less
sensitive skin would be advised to begin with the clay mask which removes impurities
in the skin and opens up the pores prior to applying the peel solution. These peels are
usually performed in a series of five to six at an interval of two to three weeks. There
is usually minimal flaking of the skin.
If you are interested in the benefits of a chemical peel, the most appropriate peel will
be recommended for your skin type.

1. What is the difference between Botox and filler
agents such as Restylane® and Artecoll®?
Botox® works quite differently from the filler agents
such as Restylane®, Perlane® and Artecoll®. Botox® is
a muscle relaxant. It is used mainly to treat frown
lines, crow’s feet and horizontal forehead lines.
These lines are all caused by underlying hyperactive
muscles. When the muscles are
no longer able to function, the
lines or furrows in the overlying
skin are dramatically softened.
Filler agents are gels which are
injected under lines, folds or
wrinkles to plump them up and
fill them in. They can be used
in most areas of the face
including in and around the lips.
2. What anti-aging skin care products can I use if I
have dry, sensitive skin and I can’t tolerate some of
the more irritating products such as tretinoin
(Retin-A®) ?
Many of the anti-aging skin care products such as
tretinoin (Retin-A®) and glycolic acid are quite
irritating to those with sensitive, dry skin. There are
some products, however, which do provide anti-aging
benefits without causing the associated redness and
irritation in most people.
Gluconolactone is similar to glycolic acid
(Neostrata®, Reversa®) but it is more moisturizing
and less irritating. Like the glycolic acid products, it
causes exfoliation of the superficial skin cells. This
reveals the younger, smoother skin below which
gives the skin a luminosity and revives it’s vibrancy.
Also, by removing the dead surface skin cells, all
other products that are used are better able to
penetrate the skin and are, therefore, more effective.
Gluconolactone should be applied in the morning.
Kinetin, the active ingredient in Kinerase®, is a
natural substance which slows down the appearance
of aging in plant cells. It has been shown to work
similarly to Retin-A® to improve the skin’s texture
and appearance while providing excellent
moisturization. It comes as a cream (for drier skin)
and a lotion (for oilier skin). It can be used twice
per day.
3. There is a lot of discussion, recently, about the risk
of blood clots on flights. What are blood clots and
how can their risk be minimized during airline
travel?
A “blood clot” or deep venous thrombosis occurs
when the blood inside the deep veins of the legs
coagulates and blocks the blood flow returning from
the lower legs back to the heart. This leads to pain,
swelling and warmth in the affected leg. The danger
of blood clots occurs when the coagulated blood
breaks free and travels to the lungs. A blood clot in
the lungs is called a pulmonary embolus. When the
clot blocks blood flow in the lungs, it is a lifethreatening emergency. However, the large majority
of blood clots in the legs never move to the lungs.
There are many tests to diagnose blood clots
including duplex ultrasounds, a special blood test
and lung scans. Blood clots are treated with blood
thinners.
Longer flights have been associated with blood clots
in an extremely tiny percentage of passengers.
Sitting in a cramped position and dehydration are
thought to be the main causes of these clots. The best
ways to prevent blood clots on flights is to move the
legs around as much as possible, drink plenty of
non-alcoholic beverages and wear compression
hosiery. (A prescription can be obtained from Dr.
Gilpin.) It is controversial as to whether or not
aspirin also decreases the risk.
People who are at a higher risk of developing a
blood clot may also require blood thinners for
flights. This includes some of those who have had a
previous clot. If you are concerned, check with your
family doctor, hematologist or Dr. Gilpin.
Vein injections (sclerotherapy) should be avoided
approximately three weeks before and after longer
flights.
The Zeno™ is a cell phone-sized device which has recently been approved in
Canada for the treatment of mild to moderate acne. It has a built-in computer
which precisely controls low level heat that can be directly applied to an
individual acne lesion.
The heat is applied to a pimple for two and a half minutes, two to three times, in a twenty-four
hour period. Clinical studies have shown that ninety percent of lesions showed improvement
or resolved in one day and that an average pimple will clear up two days sooner than normal.
There are no significant side effects.
