Clinical Case Submissions
CPA Clinical Specialty Program
Clinical Case Submission:
Candidates will write a narrative including all of the following information, or use the template
provided for three (3) clinical cases.
Please be thorough but brief.
Each clinical case has a maximum length of 5 pages.
Components of the each clinical case must include:

Description of the patient/client
o Personal factors ( e.g. age, gender etc, employment, marital status,
accommodation, , personal interests)

Health Status
o Presenting problem, diagnosis and relevant diagnostic tests
o History ( including any medical conditions)
o Medications
o Functional abilities/levels

Patient/client goals

Physiotherapeutic assessment and findings
o Tests, factors for consideration etc

Physiotherapy diagnosis

Physiotherapeutic interventions
o Selection and rationale
o Treatment progression
o Outcomes / Evaluation

Discharge planning (as applicable)

Information regarding the role of other health care providers (if involved)

Supplemental information as appropriate to assist in the description of this care of this
individual and the role of the physiotherapist specialist candidate
