MDSC 683 CANCER BIOLOGY COURSE MODULE SIGN-UP FORM
Please choose one course below, and indicate which three modules you wish to take for this course. Include your
name, student ID number, and email address, and email this form to Dr. Aaron Goodarzi [email protected].

Submit a separate form for each course you plan to take- you may not take the same module for more
than one course. Appropriate modules offered by the Immunology Research Group may be used for
electives upon approval by the course and module coordinators- see www.ucalgary.ca/irg/education
for a description of these modules.

Please submit this form no later than 1 month before the beginning of the semester (i.e. by August 1st
for Fall and by December 1st for Winter. Modules need a minimum enrollment of four students to take
place, and are generally capped at maximum of twelve students (six, in the case of Module A2, as this
has a laboratory component).

After submitting this form you will receive permission to register online for the course. The course is a
full year course and you must register (via your Student Centre) for both fall semester and winter
semester.

The final course grade for each student will be based on the standards set by FGS.
