Thank you for renting your costume from Arts Live Theatre! Rental fees go to support Arts Live Theatre
programming!
We ask costumes be returned in the condition received, and be returned by the due date listed below. A
late charge of $10 per day the costume is late in being returning will be accessed. A deposit of $25 per
costume is required and refundable when costume is returned.

In renting from Arts Live Theatre, I agree to the following:
 I understand Arts Live Theatre is providing a costume to me on a RENTAL basis. I may keep this
costume until the due date listed below. Late returns will be accessed a $10 per day late fee.
 Returned costumes will be in the condition consistent with the way I received them. If damage
or loss occurs prior to return, I agree to pay all costs for replacement at the retail value plus all
attorney and/or collection agency fees.
 Damage is considered to have occurred by: tearing, burning, cutting, make-up stains, liquid/food
stains, or marks, removal of buttons, medallions, badges or other kinds of trim. Any such
evident on a costume will be considered damaged.
 If items require additional cleaning there will be an additional $25 fee. If we are unable to get an
item cleaned it will be considered damaged and you will be responsible for replacement cost.
DO NOT try to clean our costumes yourself.
 I understand all rentals are final and there are no refunds.
 Failure to return a costume on the date due back will authorize the charge for replacement of
costume to your credit card.
 By accepting these terms you authorize your credit card on file be charged for all fees incuured.
 If paying by check, a separate check in the amount of the deposit will be required. When the
costume is returned, the deposit check will be returned.
