Background Information:
Recent clinical research in Queens Central Hospital, Blantyre, Malawi has demonstrated a high
mortality related to sepsis. Multiple factors are at play: limited human and other resources; late
presentation to hospital; uncertain diagnostics. One further, poorly understood, facet is that of
supportive management including the use of intravenous fluids. Clinical practice is this setting has no
well-established guidelines.
We wish to develop a detailed understanding of fluid management for this patient group. This
includes documenting how and when fluids are prescribed and administered, and how individual
patients respond to them.
Summary of Role:
The Clinical Research Fellow will be based in Blantyre, Malawi. They will be integrated into an
existing team of researchers, but perform a distinct role within this: the Fellow will document fluid
management for patients attending the Emergency Centre, from their earliest interface with the
research team until they leave hospital. Most activity will occur within the first six hours of
management, where frequent observations (but no direct change to clinical care) will be made. The
Fellow will take part in wider research activities as directed by the line manager (Dr Jamie Rylance)
and principal investigator of the study (Dr Joe Lewis); see below.

Screening and recruitment of research participants (in the Emergency department)

data QC and analysis

study documentation preparation and completion

developing new data capture documents

other clinical and administrative activities to support the PI as they arise
