Inform the patient to contact the renal department before taking any new meds, either prescribed or over the counter. Ask the patient to avoid any vitamins, herbal remedies, diet supplements, or weight loss therapies. Alternative medicine is a fact of life in our society. If the patient insists on use, urge waiting until after discussion with the renal physician. Patient Education Principles Ongoing throughout the CKD continuum Education is based on CKD stage Individual plan, focused on priorities dictated by the CKD stage Consider the patient’s perceived need Evaluate what the patient may already know Learning should be patient-centered and an active process Patient Education Principles Allow for independent, self-directed learning Motivate learning by focusing on the benefits (such as increased control, decreased anxiety, and informed decision making ability) Goals of education include increased compliance with treatment plan and ultimately limiting disease progression Options Education and Preparation Presentation should be an objective and non biased presentation of all available options to replace renal function Based on patient’s readiness to learn Based on rate of decline in renal function Option of no treatment should be included Patient directed decision Options Education and Preparation Encourage family member involvement in the decision making process Provide written materials, videos, and Web sites for review at home Provide a list of available kidney disease organizations Encourage dialysis unit visit to meet staff and possibly other patients Treatment decision can be changed Pre-Dialysis Education Understanding Treatment Options Where are the kidneys? Barriers to pre-dialysis education: time restrictions with patients old habits old thing---if we don’t talk about it, it is not happening inadequate interaction skills lack of feedback on current performance low confidence in new approaches Reactions pre-dialysis Term “end stage renal disease” is frightening Fear of death develops Stress of decisions about therapy Stress of learning and adhering to dietary and medical regimens Deny dialysis is needed Fear loss of job, income, lifestyle Pre-dialysis Education Primary Goal: To provide information about choices of therapy Secondary Goals: Help patient accept RRT • Realize that they are choosing the therapy to begin with, but that many will eventually have all therapies at different times in order to prolong their lives and maintain their quality of life • May choose conservative therapy (no dialysis or transplant)—we need to support them and continue to provide non-judgmental care Help integrate RRT with patient’s life Avoid admission for emergent start of dialysis Avoid temporary HD access Summary Preparation dialysis education is an important patient right. Formats vary--it is not clear what’s best Several studies identify benefits of predialysis education, including increasing the likelihood of a patient choosing a home therapy.
