Important Information for Student Athletes
For the 2016-17 school year, all students who wish to participate in athletics must enroll in Appryse prior to try-out
dates. Appryse is a web based records management system used to track medical history, physical forms and contact
information. Appryse allows parents to enter all information online as well as upload required physical forms. The
system provides improved safety for our athletes and improved privacy for our families. Through Appryse, parents can
easily update their child’s information if it changes. In the event of a sports related injury, trainers and coaches can
notify parents immediately by phone, text, and email.
To create an Appryse account, please follow these steps:
1. Use this link: https://www.lschs.org/athletics/health-safety which can also be accessed at La Salle’s
website under the Health and Safety tab on the Athletics page. Please create only one account per
student.
2. Once you create your account, you will be able to print a physical form (sample show) that can be
faxed directly to Appryse. The form contains a bar code personalized to your account. The fax number
for Appryse is listed at the top right corner. Please note: PIAA forms will not be accepted by Appryse
or La Salle College High School. Students must submit an Appryse physical form.
3. Please fill out the parent contact
information accurately. This is the
information that will be used to notify you
in the event of an injury.
4. Print your form and bring it to your son’s
physician. Ask the office to complete and
fax the form to the number at the right top
corner. PHYSICALS MUST BE PERFORMED
AFTER MAY 31, 2016 TO BE VALID. They are
good for one calendar year. If your son plays
more than one sport, Section 7 of the PIAA
physical form must be completed prior to
try-outs for additional sports.
