Basketball Camp
Date: Monday, May 23rd-Wednesday, May 25th
Where: CBMS Gymnasium
For: Girls who are entering grades: 3rd, 4th, 5th and 6th (2016-2017)
Time: 3rd and 4th grade: 3:30-5:00pm, 5th and 6th grade: 5:15-6:45pm
Camp Costs:
The cost of the Youth Camp is $30.00, $50.00 per family. The fee covers the cost of camp
and a camp T-shirt. Camp instruction will be lead by the Middle School girl’s basketball
coaches, Coach Watermann and Coach Ensor. Please make sure to drop off and pick up
all camp participants at the back entrance of the Middle School.
Equipment Needed: Everyone will need to wear proper basketball attire: T-shirt, shorts, and
basketball/tennis shoes. Water and Gatorade will be provided.

Camp Registration:
Fill out the registration form, include the camp fee, and turn into Sara Haskell at CES or
Kimberly Morris at CIS by Wednesday, May 11th.

Parental Consent:
I approve of my daughter’s participation in the youth basketball camp and certify that she is in
good health and able to participate in the activities. If medical attention is required while
attending, I give permission for such care. I further acknowledge that the Centralia Schools or
coaching staff will not be liable for any damage or injuries sustained at the camp.
