JOB DESCRIPTION POSITION TITLE: Coordinated Care Liaison DEPARTMENT: Community Services & Asset Development HOURS PER WEEK: 37.5 (full-time) LOCATION: To be determined STATUS: Non-exempt SUPERVISOR: Coordinator of Community Collaborations & Asset Development POSITION SUMMARY The Coordinated Care Liaison plays an important role in carrying out the mission of the agency by promoting better coordination of care between social, mental health, and medical services, with the goal of improving patients’/participants’ outcomes. The position builds on the many current initiatives addressing the priority on coordination of services identified in the Community Health Needs Assessment. The CCL brings people from many levels in the medical, mental health, and social service sectors together with representatives of patients/participants to improve coordination of care and to position the community to respond successfully to funding opportunities as they arise. Systems development will rely on data as a means of identifying intervening variables (also sometimes referred to as risk and protective factors) that can effectively move the dial on desired outcomes. This is a community organizing project that will mobilize people with varying perspectives to meet shared goals related to improved treatment and wellness for the residents of Franklin County. ESSENTIAL QUALIFICATIONS Associates degree is required. Three to five years work experience in medical settings; familiar with health care operations and systems required. Experience in collaborating with diverse sectors of the community. Experience in focus group facilitation. Additional related education may be substituted for one year of experience or Additional directly related experience may be substituted for education. Human service and public health experience a plus. Knowledge of Franklin, Hampshire & N. Quabbin region agencies and resources preferred. SKILLS REQUIRED: Demonstrated ability to bring people of varying perspectives together to develop shared solutions. Commitment to systems change that will enable people living in conditions that do not support wellness to receive holistic medical and social service support. Understanding of medical and social services systems and how they do and could intersect for the benefit of all, especially vulnerable populations. Advanced organizational and analytical skills (quantitative and qualitative). Excellent group facilitation skills. Excellent oral and written communication skills. Ability to pose strategic questions and serve as a thinking partner who formulates alternate approaches. Ability to work independently with minimal supervision, and as a constructive member of a team. Exceptional time management skills with ability to multitask; ability to work well under pressure, prioritize workload, and meet tight deadlines. Strong attention to detail as well as ability to be guided by “big picture” strategic priorities and goals. Creative and visionary approach to program development. Mastery of internet searches, email, online applications, Microsoft Office Suite software such as Word, Access, Excel and other analytical tools. ESSENTIAL RESPONSIBILITIES Participate in local community groups that coordinate care among social services and other sectors, e.g. North Quabbin Community Coalition, Franklin County Resource Network, and Communities That Care Coalition. Become and remain familiar with the work of the CHART grant through Baystate Franklin Medical Center (BFMC), the BFMC Community Benefits Advisory Council, Franklin County Perinatal Support Coalition, Opioid Task Force of Franklin County, etc. Learn about area social service, mental health treatment, and medical resources and contribute to maintaining the iCarol information and referral software so that it is responsive to the needs of both social service and medical providers. Facilitate meetings/conversations/focus groups with key social service and medical providers to explore, map and understand existing relationships, referral and follow-up practices, and opportunities for and barriers to collaboration. Determine best methods for hearing from patients/clients/consumers about their perception of the barriers to coordinated care and how they navigate them (e.g. focus groups, surveys), and follow through to gather and interpret information. Recruit and develop a Leadership Team of consumers and key decision-makers to advise and promote the project, set priorities, and develop work plans. Convene and facilitate a group of local Community Health Workers to provide mutual support, crossfertilization of ideas, and input into systems development. Identify gaps in outreach materials; develop and distribute new materials based on expressed needs of medical providers and consumers. This could include consultation with The Literacy Project about level of language complexity and knowledge base of consumers. With medical practices, social service providers, and consumers, identify and/or help design mechanisms for collecting and analyzing patient feedback on their experience accessing health and social support services, and use this feedback in project development. Required to travel throughout region. PHYSICAL DEMANDS AND WORKING CONDITIONS: Sitting for long periods of time. Working in office environment. Some bending and stretching required. Extensive use of telephone required. Manual dexterity required for use of calculator and computer keyboard. Must be able to lift from 20 – 30 lbs Must have reliable source of transportation that ensures timely participation in meetings in many locations Specific vision abilities required by this job include vision, color vision and the ability to adjust focus. The work environment includes indoor office environments or comparable spaces, and community spaces, with occasional exposure to outdoor weather when traveling to outreach or meeting sites. The noise level varies by site.
