Thank you for taking part in this survey, the information that we collect is important.
It is important that you understand that the survey is not as thorough as a normal examination
with your own dentist and it is difficult to examine all areas of the mouth in the same way.
In this survey our policy is to inform your family dentist or your doctor of any ulcers or inflamed
areas. There is an area like this in your mouth and because I am not sure exactly what it is I
would like to arrange for your dentist or doctor to look at this.
If you do not want your dentist or doctor to be informed it is important that you arrange to have
the area checked by a doctor or dentist in the near future.
A list of dentists is attached.
